醒脑开窍针刺法治疗椎-基底动脉综合征伴眩晕患者疗效及对脑血流动力学影响

Study on Effect of Xingnao Kaiqiao Acupuncture on Vertebrobasilar Syndrome with Vertigo and Its Influence on Cerebral Hemodynamics

目的探讨醒脑开窍针刺法对椎-基底动脉综合征伴眩晕患者的疗效及脑血流动力学的影响。方法将2017年1月—2019年11月椎-基底动脉综合征伴眩晕患者93例,随机分组,对照组45例采用氟桂利嗪5 mg/d治疗;观察组48例采用醒脑开窍针刺法治疗,持续治疗4周为1个疗程。比较两组间的临床疗效,记录大脑中动脉(middle cerebral artery,MCA)、椎动脉(vertebral artery,VA)和基底动脉(basilar artery,BA)的平均血流速度,测定全血黏度高切、全血黏度低切、血浆黏度(plasma viscosity,PV)及纤维蛋白原(fibrinogen,FIB)水平。结果对照组有效率77.78%(35/45)低于观察组93.75%(45/48),差异有统计学意义(P<0.05)。与对照组比,观察组治疗后MCA、VA及BA的脑血流速度相对较高,差异有统计学意义(P<0.05)。与对照组比,观察组治疗后全血黏度高切、全血黏度低切、PV、FIB水平相对较低,差异有统计学意义(P<0.05)。治疗期间无任何不良反应出现。结论醒脑开窍针刺法对椎-基底动脉综合征伴眩晕患者的疗效显著,可改善脑血流动力学及血液流变学,且安全性高。

Objective To investigate the effect of Xingnao Kaiqiao acupuncture on vertebrobasilar syndrome with vertigo and the influence of cerebral hemodynamics.Methods 93 cases of vertebrobasilar syndrome with vertigo from January 2017 to November 2019 were randomly divided into two groups:45 cases in the control group were treated with flunarizine 5 mg/d,48 cases in the study group were treated with Xingnao Kaiqiao acupuncture,and the course of treatment was 4 weeks.The clinical efficacy was compared between the two groups.The mean blood flow velocity of middle cerebral artery(MCA),vertebral artery(VA)and basilar artery(BA)were recorded.The whole blood viscosity high shear,whole blood viscosity low shear,plasma viscosity(PV)and fibrinogen(FIB)levels were measured.Results The effective rate of the control group was 77.78%(35/45),which was lower than that of the study group(93.75%,45/48)(P<0.05).Compared with the control group,the levels of cerebral blood flow velocity of MCA,VA and BA in the study group were relatively higher after treatment,and the difference were statistically significant(P<0.05).Compared with the control group,levels of whole blood viscosity high shear,whole blood viscosity low shear,PV,FIB in the study group were lower,the difference was statistically significant(P<0.05).No adverse reactions occurred during the treatment.Conclusion Xingnao Kaiqiao acupuncture has significant effect on vertebrobasilar syndrome with vertigo,which can improve cerebral hemodynamics and hemorheology,and has high safety.
