加味济川煎对血液透析便秘患者的疗效及对血清白蛋白和胆固醇水平的影响

Modified Jichuanjian(加味济川煎)for Hemodialysis Patients with Constipation and Its Effect on Serum Levels of Albumin and Cholesterol

目的研究加味济川煎对血液透析便秘患者的疗效及对血清白蛋白和胆固醇的影响。方法选择2016年1月—2020年1月该院肾内科收治的61例血液透析便秘患者,随机分为两组。对照组的31例血液透析便秘患者服用乳果糖口服液,每次15 mL,2次/d,总计给药1个月;观察组的30例血液透析便秘患者联合服用加味济川煎,总计给药1个月。结果观察组血液透析便秘患者的有效率(96.67%,29/30)明显高于对照组(74.19%,23/31)(P<0.05);治疗后,观察组患者的血清白蛋白的升高程度明显高于对照组(P<0.05),且观察组的胆固醇下降程度明显优于对照组(P<0.05);治疗后,两组的排便时间、排便周期、排便无力、腹胀程度、腰膝酸软、精神状态、口干程度、自汗程度、头重程度积分均明显降低(P<0.05),且观察组的排便时间、排便周期、排便无力、腹胀程度、腰膝酸软、精神状态、口干程度、自汗程度、头重程度积分明显低于对照组(P<0.05);治疗后3个月,观察组血液透析便秘患者的复发率为16.67%(5/30),明显低于对照组血液透析便秘患者的38.71%(12/31)(P<0.05)。结论加味济川煎能有效改善血液透析便秘患者的症状,可有效降低血清胆固醇水平,并可升高血清白蛋白水平,进而有效促进胃肠道的动力,减少复发,值得推广。

Objective To study the therapeutic effect of Jiawei Jichuanjian(加味济川煎)on patients with constipation during hemodialysis and its effect on serum albumin and cholesterol.Methods Sixty-one patients with constipation of hemodialysis who were admitted to the Department of Nephrology of our hospital from January 2016 to January 2020,were divided into two groups rando mLy.In the control group,31 patients with hemodialysis constipation took lactulose oral solution,15 mL for each time,twice a day,a total of 1 month.In the observation group,30 patients with hemodialysis constipation took Jiawei Jichuan Decoction for a total of 1 month.Results The effective rate(96.67%,29/30)of the observation group was significantly higher than that of the control group(74.19%,23/31)(P<0.05).After treatment,the serum albumin of the observation group was significantly higher than that of the control group(P<0.05),and the cholesterol reduction of the observation group was significantly better than that of the control group(P<0.05).After treatment,the defecation time,defecation cycle,defecation weakness,abdominal distension,waist and knee weakness,mental state,dry mouth,spontaneous sweating and head weight were all significantly reduced(P<0.05).Defecation time,defecation cycle,defecation weakness,abdominal distension,waist and knee weakness,mental state,dry mouth,spontaneous sweating and head weight score were significantly lower than those of the control group(P<0.05).Three months after treatment,the recurrence rate of constipation in the observation group was 16.67%(5/30),which was significantly lower than that of control group(38.71%,12/31)(P<0.05).Conclusion Modified Jichuanjian could effectively improve the symptoms of patients with constipation during hemodialysis,reduce the serum cholesterol level,and increase the serum albumin level,so as to effectively promote the power of the gastrointestinal tract and reduce recurrence.It was worthy of promoting.
