蜈蚣败毒饮对成人不同年龄组寻常性银屑病患者病证影响的相关性研究

Correlation Study of Wugong Baidu Decoction(蜈蚣败毒饮)on Disease and Syndromes of Patients with Psoriasis Vulgaris in Different Adult Ages

目的基于中医学血分辨治理论,观察蜈蚣败毒饮治疗成人不同年龄组寻常性银屑病患者的临床疗效,同时对不同年龄段中年龄参数与皮损PASI变化值之间的相关性进行分析。方法将400例寻常性银屑病患者随机分为治疗组和对照组,除去脱落病例,共完成实际病例数395例,根据患者年龄的不同将其分为青年、中年、老年,根据患者证型的不同将其分为血热证、血燥证、血瘀证,如此将每例患者入组相应的治疗组别、年龄组别、证候组别。其中治疗组口服中药制剂蜈蚣败毒饮,对照组口服复方甘草酸苷片,采用PASI评分法分年龄、分证型对各组患者的皮损疗效进行分析,同时分年龄、分证型对蜈蚣败毒饮所干预患者的年龄参数和PASI变化量之间的相关性进行评价。结果青年组以血热证患者居多,老年组以血瘀证患者居多,蜈蚣败毒饮对各年龄组中不同血分证候患者的皮损PASI分值均具有下调作用(P<0.05或P<0.01),以血热证和血瘀证作用最为显著;经蜈蚣败毒饮干预后,青年组和中年组的血热证患者皮损PASI变化量与年龄之间呈现负相关(P<0.01),中年组和老年组的血瘀证患者皮损PASI变化量与年龄之间呈现正相关(P<0.01)。结论蜈蚣败毒饮具有治疗寻常性银屑病的作用,不同的年龄组可能具有不同的优势证候,随着年龄的改变,各证候间可能存在一定的演变规律,而蜈蚣败毒饮发挥作用的机制也可因年龄及证候的不同而呈现出多靶点样效应。

Objective To observe the clinical effect of Wugong Baidu Decoction(蜈蚣败毒饮)on psoriasis vulgaris in different adult age groups based on the theory of differentiation and treatment of blood in traditional Chinese Medicine,at the same time,to analyze the correlation between the age parameters and the changes of PASI in different age groups.Methods A total of 400 patients with psoriasis vulgaris were randomly divided into treatment group and control group.A total of 395 cases were completed without shedding cases and were divided into youth group,middle age group and old age group according to the ages of patients,and also were divided into blood heat syndrome,blood dryness syndrome and blood stasis syndrome according to the different syndrome types.In this way,each patient separately was put into the corresponding treatment,age and syndrome groups.Wugong Baidu Decoction was taken orally in the treatment group,while in the control group,Compound Glycyrrhizin Tablet was taken orally.PASI scoring method was used to analyze the efficacy of skin lesions in each group according to the age groups and syndrome groups,meanwhile,the correlation study was applied to analyze the relevance between the age parameters and PASI changes of the patients who were intervened by Wugong Baidu Decoction according to the age groups and syndrome groups.Results The majority of the youth group were patients with blood heat syndrome,the majority of the old age group were patients with blood stasis syndrome.Wugong Baidu Decoction can reduce the PASI score of skin lesions of patients with different blood syndromes in different age groups(P<0.05 or P<0.01).In addition,the blood heat syndrome and blood stasis syndrome have the most significant effects.After treatment with Wugong Baidu Decoction,there were negative correlations between the changes of PASI and ages in youth and middle age patients with blood heat syndrome(P<0.01),and positive correlations between the changes of PASI and ages in middle and old age patients with blood stasis syndrome(P<0.01).Conclusion Wugong Baidu Decoction has the effect on treating psoriasis vulgaris.Different age groups may have different dominant syndromes,there may be a certain law of evolution among various syndromes with the change of ages,and mechanisms of the action of Wugong Baidu Decoction can also show multi-target effect due to different ages and syndromes.
