刘伟敬从风-虚-瘀论治肾性血尿思路探析

LIU Weijing′s Thinking on Treating Renal Hematuria from Perspective of Wind-Deficiency-Stasis

肾性血尿,即血尿来自肾脏,多指肾小球源性血尿。中医学将其归属于"尿血""溲血""溺血"等范畴。血尿为肾系疾病常见症状之一,是多种急慢性肾脏疾病主要临床表现,IgA肾病尤为多见。在该疾病中,血尿多长期伴随IgA肾病的发生发展过程中,反复发作,迁延难愈。现代医学目前鲜有特效药物,而传统医学对肾性血尿的治疗积累了宝贵经验,认为其基本病机为本虚标实,以脾肾亏虚为本,以风、热、瘀、毒为标,刘伟敬教授结合中医理论及多年的临床实践经验,总结出血尿的关键病因为伏风内扰、瘀血阻滞肾络、脾肾不足等。因此治疗上刘教授提出从风、瘀、虚论治,结合脏腑辨证,以达扶正祛邪,标本同治的目的。

Renal hematuria, that is hematuria from the kidney, refers to glomerular hematuria.In traditional Chinese medicine, it belongs to the category of “hematuria”“blood in urine”“bloody urine” and so on.Hematuria is one of the common symptoms of kidney diseases, and is the main clinical manifestation of various acute and chronic kidney diseases, especially IgA nephropathy.In this disease, IgA nephropathy is often accompanied by hematuria for a long time.In the process of development, hematuria often occurs repeatedly and cannot be cured.At present, there are few effective drugs in modern medicine, but traditional Chinese medicine has accumulated valuable experience in the treatment of renal hematuria.It is believed that the basic pathogenesis is based on the deficiency of spleen and kidney, and the wind, heat, blood stasis and toxin are the pathological products.Professor LIU Weijing combined with the basic theory of traditional Chinese medicine and many years of clinical practice experience, summarized the key causes of hematuria, such as spleen and kidney deficiency, internal disturbance of latent wind and blood stasis blocking kidney collaterals.Therefore, Professor LIU proposed that the treatment should be based on the wind, blood stasis and deficiency, combined with the Zang-Fu organs syndrome differentiation, so as to achieve the purpose of strengthening the body and removing pathogenic factors and treating the specimens together.
