基于GC-MS的肝肾亏虚型膝骨关节炎患者的代谢组学研究

摘要 目的运用GC-MS代谢组学方法研究肝肾亏虚型膝骨关节炎(KOA)患者和健康人群尿液代谢产物差异,探讨疾病发生机制。方法选取25例明确诊断为肝肾亏虚型膝骨关节炎的患者作为病例组(模型组),另选取25例中医体质为平和质的健康体检者作为正常对照组(对照组)。对试验人群的尿液样本进行GC-MS检测并对数据进行多元统计分析。结果基于VIP>1和P<0.05的原则筛选得到以下差异标志物,相比于健康体检者,KOA患者尿液中葡萄糖、甘露醇、核糖、琥珀酸含量显著升高(P<0.05),酒石酸、苏氨酸、柠檬酸、肌醇含量显著降低(P<0.05);通过代谢通路富集分析,发现肝肾亏虚型膝骨关节炎患者淀粉和蔗糖代谢、甘油酯代谢、丙酮酸代谢、三羧酸循环和磷酸肌醇代谢发生紊乱。结论肝肾亏虚型膝骨关节炎主要影响机体能量代谢和免疫调节能力。 Objective To study the pathogenesis of knee osteoarthritis of liver and kidney deficiency syndrome by researching the difference of urine metabolites between patients and healthy people by GC-MS metabolomics.Methods Twenty-five cases of liver and kidney deficiency type of knee osteoarthritis were selected as the case group(model group),and 25 cases of healthyphysical examination as the normal control group(control group).The urine samples of the test population were detected by GC-MS and the data was analyzed by multivariate statistical analysis.ResultsThe differential markers were screened based on theprinciple of VIP>1 andP<0.05.Compared with the healthy people,the contents of amino malonic acid,threonine,proline and ino-sitol were significantly decreased(P<0.05).The contents of glucose,mannitol,ribose and succinic acid were significantly in-creased(P<0.05).The contents of tartaric acid,threonine,citric acid and inositol were significantly decreased(P<0.05).Throughthe analysis of metabolic pathway enrichment,it was found that starch and sucrose metabolism,glyceride metabolism,pyruvate me-tabolism,tricarboxylic acid cycle and inositol phosphate metabolism in KOA patients were disordered.ConclusionThe pathogene-sis of liver and kidney deficiency type of knee osteoarthritis affected the energy metabolism and immune regulation.
