何学红基于“益气泄浊通络法”运用肾衰方治疗慢性肾衰竭临床研究

Clinical Study on HE Xuehong′s Treatment of Chronic Renal Failure with Shenshuai Recipe(肾衰方)Based on“Benefiting Qi and Purging The Turbid and Dredging Collaterals Method”

目的对何学红教授基于"益气泄浊通络法"治疗慢性肾衰竭的临床研究进行经验总撷,探究肾衰方治疗慢性肾衰竭的临床疗效。方法研究对象为2018年3月—2020年3月该院肾内科收治的112例慢性肾衰竭患者,随机分为观察组56例和对照组56例。对照组采用常规西医治疗,观察组在常规治疗的基础上联合肾衰方治疗,两组治疗时间均为4周。比较两组患者尿素氮(BUN)、肌酐(SCr)、β2-微球蛋白(β2-M)及肾小球滤过率(eGFR)肾功能指标,白细胞介素-6(IL-6)、白细胞介素-10(IL-10)、超敏C反应蛋白(hs-CRP)及人核转录因子(NF-κB)炎性因子表达含量,中医症状积分及临床疗效。结果两组患者BUN、SCr、β2-M均降低,eGFR均提高(P<0.05),经组间比较观察组上述指标改善较对照组显著(P<0.05);两组患者IL-6、IL-8、CRP及NF-κB均降低(P<0.05),经组间比较观察组上述指标改善较对照组显著(P<0.05);两组中医症状积分均降低(P<0.05),经组间比较观察组积分降低程度显著(P<0.05);观察组临床总有效率明显优于对照组[94.64%(52/56)vs 71.43%(40/56)],差异具有统计学意义(P<0.05)。结论基于何学红教授"益气泄浊通络法"为指导原则的肾衰方可起到益气活血、泄浊通络的功效,在治疗慢性肾衰竭方面取得较为满意的临床疗效,显著缓解或消除临床症状,改善肾功能,调节炎性因子表达含量,值得临床应用。

Objective This paper summarized Professor HE Xuehong′s experience on treating chronic renal failure based on“benefiting Qi and purging the turbid and dredging collaterals method”,and explored the clinical efficacy of Shenshuai Recipe(肾衰方)on treating patients with chronic renal failure.Methods We collected the information of chronic renal failure patients admit⁃ted to the Department of Nephrology in our hospital from March 2018 to March 2020.A total of 112 patients were randomly divid⁃ed into two groups(control group:group A vs.treatment group:group B).Fifty-six patients in the group A were treated with con⁃ventional western medicine treatment(Schedule A).Fifty-six patients in the group B were treated with Shenshuai Recipe plus Schedule A.Treatment course was 4 weeks.Urea nitrogen(BUN),serum creatinine(SCr),β2-microglobulin(β2-M)and esti⁃mated GFR(eGFR),expression levels of inflammatory factors such as interleukin-6(IL-6),interleukin-10(IL-10),hyper⁃sensitive C-reactive protein(hs-CRP)and nuclearfactor-kappa B(NF-κB),TCM syndrome scores before and after treatment and the clinical effects were compared.Results After treatment,the levels of BUN,SCr andβ2-M were decreased and the levels of eGFR increased(P<0.05).The improvement degree of the above indexes in the group B was significantly better than that in the group A(P<0.05).In both groups,the levels of IL-6,IL-8,CRP and NF-κB were decreased.The improvement degree of the above indexes in the group B was significantly better than that in the group A(P<0.05).The TCM syndrome scores in both groups were decreased(P<0.05).The improvement degree in the group B was significantly better(P<0.05).The clinical total effective rate in group B(90.32%,52/56)was significantly higher than that in group A(70.97%,40/56)(P<0.05).Conclu⁃sion Shenshuai Recipe,guided by Professor HE Xuehong′s“benefiting Qi and purging the turbid and dredging collaterals meth⁃od”,has the efficacy of invigorating Qi and promoting blood circulation,relieving turbidity and dredging collaterals,and has a⁃chieved satisfactory clinical efficacy in treating chronic renal failure.It can effectively relieve patients′clinical symptoms,improve renal function indexes and regulate the expression of inflammatory factors,which is worthy of clinical application.
