仙方活命饮加味方加减对慢性骨髓炎术后患者血清炎症因子的影响

Effect of Modified Xianfang Huoming Decoction(仙方活命饮加味方)on Serum Inflammatory Factors in Patients with Chronic Osteomyelitis

目的观察仙方活命饮加味方对于慢性骨髓炎术后患者血清炎症因子的影响。方法选取该院2016年5月—2019年5月收治的慢性骨髓炎术后患者106例,根据随机数字表法分成对照组(n=53)、观察组(n=53)。对照组于术后给予常规治疗,观察组在对照组基础上加用仙方活命饮加味方。比较两组术后恢复效果,包括骨愈合时间、术后下地时间、首次完全负重时间以及住院时间。分别在治疗前、治疗1周后评价两组患处疼痛、关节僵硬、脉沉细、舌质黯淡积分的变化。于治疗前、治疗1周后采血检测血清白细胞介素-6(IL-6)、白细胞介素-8(IL-8)、肿瘤坏死因子-α(TNF-α)水平,记录住院期间并发症发生率。结果观察组骨愈合时间、术后下地时间、首次完全负重时间以及住院时间显著低于对照组(均P<0.05)。两组治疗后患处疼痛、关节僵硬、脉沉细、舌质黯淡积分及总分低于治疗前,且观察组显著低于对照组(均P<0.05)。两组治疗后血清IL-6、IL-8、TNF-α水平低于治疗前,且观察组低于对照组(均P<0.05)。两组皮肤软组织嵌顿、断端愈合不良、切口感染以及再生骨矿化不良发生率比较差异无统计学意义(均P>0.05)。结论在常规治疗基础上加用仙方活命饮加味方,能进一步促进患者功能恢复,减轻炎症,临床应用价值较高。

Objective To observe the effect of Modified Xianfang Huoming Decoction(仙方活命饮加味方)on serum inflam⁃matory factors in patients with chronic osteomyelitis after surgery.Methods A total of 106 postoperative chronic osteomyelitis pa⁃tients admitted to our hospital from May 2016 to May 2019 were selected and divided into control group(n=53)and observation group(n=53)according to the random number table method.The control group was given conventional treatment after operation,and the observation group added Modified Xianfang Huoming Decoction on the basis of the control group.The postoperative recov⁃ery effects of the two groups were compared,including bone healing index,bony results,and functional results.The scores of pain in the affected area,joint stiffness,thin pulse,and dim tongue were evaluated before treatment and 1 week after treatment.Blood was collected before treatment and 1 week after treatment to detect serum levels of interleukin-6(IL-6),interleukin-8(IL-8)and tumor necrosis factor-α(TNF-α),and the incidence of complications during hospitalization was recorded.Results The bone healing index of the observation group was significantly higher than that of the control group,and the excellent and good rates of bony results and functional results were significantly higher than those of the control group(all P<0.05).After treatment,the pain in the affected area,joint stiffness,narrow pulse,dim tongueand total score were lower in the two groups than those before treatment,and those of the observation group were significantly lower than those of the control group(all P<0.05).Serum levels of IL-6,IL-8 and TNF-αafter treatment in the two groups were lower than those before treatment,and those of the observation group were lower than those of the control group(all P<0.05).There was no significant difference in the incidence of skin and soft tissue incarceration,poor healing of broken ends,incision infection or poor regenerated bone mineralization between the two groups(all P>0.05).Conclusion Modified Xianfang Huoming Decoction on the basis of conventional treatment can further pro⁃mote the functional recovery of patients and reduce inflammation,which has a higher clinical application value.
