基于扶阳思想探析唐农次第治疗大肠癌经验

Analysis of TANG Nong′s Experience in Treatment of Colorectal Cancer Based on Thought of Supporting Yang

基于钦安卢氏一派以阳主阴从为核心的扶阳思想,立足内阳外阴本体结构论,在《素问·阴阳应象大论篇》"阳化气,阴成形"理论指导下,作者认为大肠癌患者的发病机理是阳虚为本,阴成形太过,阴阳失其本位,元阳失运产生气滞、血瘀、痰浊等病理产物胶着互结于肠而形成"阳虚为本,阴实为标"的病理变化。总结火神派唐农教授治疗大肠癌的经验,以"次第治疗"为治疗准则,临证巧用姜、桂、附等中药,即先予桂枝法清宣、温通中上二焦以"去其血脉"复转中枢,继而予四逆法温固下焦,温扶元阳,后益精填髓,培元固本。使阳由下至上、由内至外层层温通,阴由上至下、由外至内层层清通,阴阳复其本位,升降有序。最终维持"阴平阳秘""阴阳自和"的生命常态,中医药对治疗肿瘤独具优势。因此,基于扶阳思想探讨大肠癌的病因病机及治疗方法,将为大肠癌的临床治疗提供依据。

Based on Qin′an LU′s thought of supporting Yang with Yang as the core,and based on the ontology of Yang inside and Yin outside,under the guidance of the theory of“Yang turns into Qi and Yin forms”in Suwen.Yinyang Yingxiang Dalun,we believe that the pathogenesis of colorectal cancer is Yang deficiency,Yin forming too much,Yin and Yang losing its standard,and the loss of Yuan Yang produces pathological products such as Qi stagnation,blood stasis and phlegm turbidity.The pathological changes of“Yang deficiency as the root and Yin excess as the standard”were formed due to the adhesion of the intestine.To summarize the experience of Professor TANG Nong of Huoshen School in the treatment of colorectal cancer,taking“the second treatment”as the treatment criterion.In clinical practice,ginger,osmanthus and Rhizoma Aconiti are used skillfully,that is to say,Guizhi is used to clear and disperse,warming and dredging middle and upper Jiao is used to“remove its blood vessels”and then Sini method is used to warm and consolidate the lower Jiao,warm and support Yuan Yang,and then benefiting essence is used to fill marrow and cultivate the root.The method is to make Yang from the bottom to the top,from the inside to the outside layer warm,Yin from the top to the bottom,from the outside to the inside layer clear,Yin and Yang return to their standard,rise and fall orderly.In order to maintain the normal life of“dynamic equilibrium of Yin and Yang”and“Yin and Yang harmony”,traditional Chinese medicine has unique advantages in the treatment of tumor.Therefore,to explore the etiology,pathogenesis and treatment of colorectal cancer based on the thought of supporting Yang will provide the basis for the clinical treatment of colorectal cancer.
