李跃华运用舒筋通络方治疗椎-基底动脉供血不足眩晕经验

LI Yuehua′s Experience on Treating Vertigo with Vertebrobasilar Insufficiency by Using Self-designed Shujin Tongluo Prescription(舒筋通络方)

椎-基底动脉供血不足眩晕是一种常见的缺血性脑血管疾病,临床表现通常以发作性眩晕为主,缠绵反复,对老年人的生活质量造成一定影响,还是许多脑血管病的促进因素。目前西医多以对症处理为主,重在治标,而中医治疗重在培元固本,远期效果好。该病属于中医"眩晕"范畴,从古至今对其病因病机说法不一。李跃华教授作为中国中医科学院西苑医院老年医学研究所副所长,老年病学专家,从医多年一直致力于老年慢性衰弱疾病的研究。她认为老年人脏腑功能减退,正气不足,阴阳两虚,其中尤以先后天之肝肾不足、脾胃衰弱为椎-基底供血不足眩晕的根本病因。基本病机是肝肾不足,气虚血瘀,清阳不升,关键在气虚血瘀。李教授遵循中医药发展规律,守正创新,在临床上运用自拟的舒筋通络方益气升阳、活血化瘀、补益肝肾,标本兼顾,收效良好。

Vertigo with vertebrobasilar insufficiency is a common ischemic cerebrovascular disease.Clinical manifestations are usually paroxysmal vertigo.It is characterized by lingering and repeated.To some extent,it has an impact on the quality of life of the elderly,and it is also the promoting factor of many cerebrovascular diseases.Now western medicine mainly focuses on symp⁃tomatic treatment,focusing on the treatment of symptoms rather than the cause,while traditional Chinese medicine focuses on rein⁃forcing the vital essence and strengthening the primordial Qi,with good long-term effect.It belongs to vertigo in traditional Chi⁃nese medicine.From ancient times to now,there are different opinions on its etiology and pathogenesis.Professor LI Yuehua as deputy director of institute of geriatrics,Xiyuan Hospital,China Academy of Chinese Medical Sciences.She has been engaged in the research of chronic debilitating diseases in the elderly for many years.She believes that the main etiology of vertigo with verte⁃brobasilar insufficiency is the decreased function of the viscera of the elderly,the positive Qi is insufficient,and the Yin and Yang are deficient,especially congenital deficiency of liver and kidney,acquired deficiency of spleen and stomach,and its basic patho⁃genesis is insufficiency of the liver and kidney,Qi deficiency causing blood stasis,lucid Yang failing to rise.The key lies in Qi de⁃ficiency and blood stasis.Professor LI follows the development law of TCM and keeps the right and innovation.Using self-de⁃signed Shujin Tongluo Prescription(舒筋通络方)to achieve benefiting Qi and raising Yang combined with promoting blood flow and removing blood stasis and tonifying the liver and kidney in clinical practice.Full of comprehensive consideration and obtained the good clinical curative effect.
