香砂养胃汤加减结合针刺疗法治疗脾胃虚弱型慢性浅表性胃炎临床疗效及安全性研究

Clinical Efficacy and Safety of Xiangsha Yangwei Decoction(香砂养胃汤)Combined with Acupuncture Therapy in Treatment of Chronic Superficial Gastritis with Spleen and Stomach Deficiency

目的探究香砂养胃汤加减结合针刺疗法治疗脾胃虚弱型慢性浅表性胃炎临床疗效及安全性。方法研究共计纳入157例脾胃虚弱型慢性浅表性胃炎患者,均由该院2017年1月—2019年9月收治,采取随机数字表法将患者分为两组,对照组患者(78例)口服瑞巴派特片治疗,观察组患者(79例)在对照组治疗基础上联合香砂养胃汤加减及针刺疗法治疗,比较两组患者疗效、治疗前后内镜检查积分变化及中医证候(胃脘隐痛、喜按喜暖、神疲乏力、食后胀满、大便稀溏、纳呆食少)积分变化、治疗后胃脘隐痛、腹胀、嗳气、反酸恶心等症状改善时间、患者不良反应及6个月后疾病复发率。结果与对照组(84.62%,66/78)比较,观察组患者总有效率(94.94%,75/79)更高,P<0.05;治疗前,两组患者内镜检查积分、胃脘隐痛、喜按喜暖、神疲乏力、食后胀满、大便稀溏、纳呆食少等中医症状积分比较,P>0.05,治疗后各组患者内镜检查积分、胃脘隐痛、喜按喜暖、神疲乏力、食后胀满、大便稀溏、纳呆食少等中医症状积分下降,观察组患者治疗后内镜检查积分、胃脘隐痛、喜按喜暖、神疲乏力、食后胀满、大便稀溏、纳呆食少等中医症状积分均低于对照组,P<0.05;观察组患者胃脘隐痛、腹胀、嗳气、反酸恶心等症状改善时间短于对照组,P<0.05;两组均未见不良反应(0.00%),P>0.05,而观察组患者6个月复发率(0.00%)低于对照组(5.13%,4/78),P<0.05。结论香砂养胃汤加减结合针刺疗法治疗脾胃虚弱型慢性浅表性胃炎疗效佳,患者恢复好,恢复快,无不良反应,6个月复发率低,值得推广及应用。

Objective To explore the clinical efficacy and safety of Xiangsha Yangwei Decoction(香砂养胃汤)combined with acupuncture therapy in the treatment of chronic superficial gastritis with spleen and stomach deficiency.Methods In the study,157 patients with spleen and stomach deficiency type of chronic superficial gastritis in our hospital from January 2017 to September 2019,by taking a random number table method,were divided into two groups.The control group(78 cases)were treated with Rebamipide Tablets orally.The observation group(79 cases)on the basis added Xiangsha Yangwei Decoction.The curative effect,changes of endoscopic examination score and TCM syndrome(epigastric pain,preferring being pressed and belching,mental fatigue,fullness after eating,loose stool and little appetite)score,improvement time of epigastric pain,abdominal distension,belching,acid reflux and nausea after treatment,adverse reactions and disease recurrence rate after 6 months were compared between the two groups.Results Compared with that of the control group(84.62%,66/78),the total effective rate of the observation group(94.94%,75/79)was higher(P<0.05).Before treatment,the comparisons of the score of TCM symptoms of the two groups,such as endoscopic examination score,epigastral pain,fatigue,swelling after eating,loose stool and less food for staying in the two groups were not significant(P>0.05).After treatment,the TCM symptom scores of endoscopy,epigastric pain,mental fatigue,fullness after eating,loose stool and anorexia were decreased in each group.Those in the observation group were lower than those in the control group(P<0.05).In the observation group,the improvement time of symptoms such as epigastric pain,abdominal distension,belching and acid regurgitation was shorter than that of the control group(P<0.05).No adverse reactions were observed in the two groups(0.00%)(P>0.05),while the 6-month recurrence rate in the observation group(0.00%)was lower than that in the control group(5.13%,4/78)(P<0.05).Conclusion The combination of Xiangsha Yangwei Decoction and acupuncture therapy in the treatment of chronic superficial gastritis with spleen and stomach deficiency has a good curative effect.The patients recover well and quickly,with no adverse reactions and have a low recurrence rate in 6 months.Therefore,it is worthy of promotion and application.
