独活寄生汤对腰椎退行性变患者椎间活动度、VAS及ODI评分的影响

Study on Effect of Duhuo Jisheng Decoction( 独活寄生汤) in Treatment of Patients withLumbar Degeneration on Intervertebral Range of Motion,VAS and ODI Scores

目的探讨独活寄生汤对腰椎退行性变患者椎间活动度、VAS及ODI评分的影响。方法将该院2016年4月—2017年11月期间诊治的腰椎退行性变患者74例,随机分对照组和观察组,分别37例,对照组采用塞来昔布0.2 g,2次/d口服;在上述治疗基础上,观察组采用独活寄生汤加减治疗,2次/d口服,早晚各1次,持续治疗4周为1个疗程。治疗前、治疗2周及治疗4周时评定患者视觉模拟量表评分(visual analogue scale,VAS)、Oswestry功能障碍指数(Oswestry disability Index,ODI)及椎间隙高度、椎间活动度(Range of motion,ROM),比较临床疗效。结果对照组有效率[78.38%(29/37)]低于观察组有效率[94.59%(35/37)],差异有统计学意义(P<0.05)。两组患者不同时间点VAS评分、ODI评分及椎间隙高度、椎间ROM比较,差异有统计学意义(P<0.05)。治疗2周对照组和观察组VAS评分(4.67±1.80)、(3.49±1.53),ODI评分为(31.02±4.60)、(27.12±5.16),椎间隙高度为(10.07±1.46)cm、(11.02±1.34)cm,椎间ROM为(6.10±0.68)°、(5.49±0.57)°;治疗4周对照组和观察组VAS评分(2.84±1.24)、(1.86±1.01),ODI评分为(26.10±4.37)、(22.31±4.19),椎间隙高度为(11.21±1.34)cm、(12.35±1.67)cm,椎间ROM为(5.26±0.55)°、(4.37±0.41)°,对照组和观察组上述指标比较,差异有统计学意义(P<0.05)。结论独活寄生汤对腰椎退行性变患者的疗效确切,可改善椎间活动度和椎体活动功能,减少疼痛,且安全性高,值得推广。

Objective To investigate the effect of Duhuo Jisheng Decoction(独活寄生汤)in the treatment of patients with lumbar degeneration on intervertebral range of motion,VAS and ODI scores.Methods Seventy-four patients with degenerative lumbar spine disease treated in our hospital from April 2016 to November 2017 were randomly divided into control group(37 cases)and observation group(37 cases).The control group was given Celecoxib 0.2 g twice a day.On the basis of the above treatment,the observation group was given Duhuo Jisheng Decoction twice a day,once in the morning and once in the evening.Four weeks was a course of treatment.Visual analogue scale(VAS),Oswestry disability index(ODI),intervertebral space height and intervertebral motion(ROM)were evaluated before treatment,2 and 4 weeks after treatment to compare the clinical efficacy.Results The effective rate of the control group was 78.38%,which was lower than that of the observation group(94.59%)(P<0.05).The VAS score,ODI score,intervertebral space height and ROM of the two groups at different time points were significantly different(P<0.05).Two weeks after treatment,the VAS scores of the control group and the observation group were(4.67±1.80)points and(3.49±1.53)points,respectively.ODI scores of the control group and the observation group were(31.02±4.60)points and(27.12±5.16)points,,respectively.The intervertebral space heights of the control group and the observation group were(10.07±1.46)cm and(11.02±1.34)cm,respectively.The intervertebral ROM values of the control group and the observation group were(6.10±0.68)°and(5.49±0.57)°,respectively.VAS scores of the control group and the observation group were(2.84±1.24)points and(1.86±1.01)points,respectively.And ODI scores of the control group and the observation group were(26.10±4.37)points and(22.31±4.19)points,respectively.The heights of intervertebral space of the control group and the observation group were(11.21±1.34)cm and(12.35±1.67)cm,respectively and the ROM values of intervertebral space of the control group and the observation group were(5.26±0.55)°and(4.37±0.41)°,respectively.All indexes in two groups had significant differences(P<0.05).Conclusion Duhuo Jisheng Decoction has definite effect on lumbar degeneration patients.It can improve intervertebral mobility and vertebral function,reduce pain and has high safety.It is worthy of popularizing.
