滋阴清热汤联合针灸治疗老年阴虚血少心神不宁型失眠

Clinical Efficacy of Ziyin Qingre Decoction( 滋阴清热汤) Combined with Acupuncture inTreatment of Elderly Insomnia of Yin Deficiency,Blood Deficiency and Restlessness Type

目的探索滋阴清热汤联合针灸治疗老年阴虚血少心神不宁型失眠的临床疗效。方法选取该院于2018年9月—2019年12月期间收治的老年阴虚血少心神不宁型失眠患者100例为研究对象,随机分为汤剂组和汤针联合组,各组50例患者。汤剂组每天口服滋阴清热汤,连续口服4周;汤针组除每天口服滋阴清热汤外,给予常规针灸治疗,隔天1次,每周3次,治疗周期为4周。比较了两组患者治疗前后临床症状积分、匹茨堡睡眠质量指数量表(PSQI)评分、临床疗效。并用视觉模拟评分法(VAS)评分评估两组患者治疗前后临床症状积分、缓解患者头疼症状的临床疗效。结果两组组患者PQSI评分差异有统计学意义(P<0.05),滋阴清热汤联合针灸组治疗失眠的总体疗效好于汤剂组(P<0.05),且睡眠相关指标比较有显著性差异。此外,VAS评分结果显示两组患者治疗前后临床症状积分有显著性差异,滋阴清热汤联合针灸组缓解患者头疼疗效明显好于汤剂组(P<0.05)。结论滋阴清热汤联合针灸治疗失眠疗效优于单纯汤剂治疗,临床上可用滋阴清热汤联合针灸治疗老年阴虚血少心神不宁型失眠。

Objective To explore the clinical effect of Ziyin Qingre Decoction(滋阴清热汤)combined with acupuncture in the treatment of elderly insomnia of Yin deficiency,blood deficiency and restlessness type.Methods A total of 100 elderly patients with insomnia of Yin deficiency,blood deficiency and restlessness type treated in our hospital between September 2018 and December 2019 were selected as the research subjects.All patients were randomly divided into the decoction group(DG)and Ziyin Qingre Decoction combined with acupuncture group(DAG),and each group had 50 patients.The DG group was orally administered with Ziyin Qingre Decoction daily for 4 weeks.In addition to the oral administration of Ziyin Qingre Decoction,the DAG group was given daily acupuncture,once every other day,3 times a week,and the treatment period was 4 weeks.The clinical symptom score,Pittsburgh sleep quality index(PSQI)score and clinical efficacy were compared between the two groups before and after treatment.The visual analogue score(VAS)was used to evaluate the clinical symptom scores before and after treatment as well as the clinical efficacy of alleviating headache symptoms.Results The differences in PQSI scores between the two groups were statistically significant(P<0.05).The overall effect of Ziyin Qingre Decoction combined with acupuncture in the treatment of insomnia was better than that in the DG group(P<0.05),and there were significant differences in sleep-related indicators.In addition,the results of the VAS score showed significant differences in clinical symptom scores between the two group before and after treatment.The effect of DAG group on headache relief was significantly better than that of the DG group(P<0.05).Conclusion The therapeutic effect of Ziyin Qingre Decoction combined with acupuncture on insomnia is better than that of Ziyin Qingre Decoction alone.Clinically,acupuncture can be used to treat elderly insomnia with Yin deficiency,blood deficiency and uneasiness type.
