推拿牵引联合冲击波治疗腰椎间盘突出症

Tuina and Traction Combined with Shock Wave in Treatment of Lumbar Intervertebral Disc Herniation

目的观察推拿牵引联合脉动仪治疗腰椎间盘突出症疗效。方法纳入2017年10月—2020年7月期间科室收治的80例腰椎间盘突出症患者进行研究,采用随机数字表法分为观察组、对照组各40例,对照组患者予以肌肉松弛剂、非甾体抗炎药治疗,观察组患者在此基础上辅以推拿牵引联合脉动仪治疗。比较两组患者临床总有效率,及治疗前、治疗2周后腰椎疼痛症状[视觉模拟评分法(VAS)、日本矫形外科协会(JOA)]评分、腰椎关节屈曲活动度、腰椎功能[功能独立性评测量表(FIM)]评分。结果治疗2周后,观察组治疗总有效率明显高于对照组(均P<0.05);两组患者VAS评分均较治疗前降低,且观察组低于对照组,JOA评分均较治疗前升高,且观察组高于对照组(均P<0.05);两组患者腰椎关节屈曲活动度均较治疗前升高,且观察组高于对照组(均P<0.05);两组患者FIM括约肌控制、自理活动、行进、转移评分均较治疗前升高,且观察组高于对照组(均P<0.05),交流、社会认知评分与治疗前比较差异均无统计学意义(均P>0.05)。结论推拿牵引联合脉动仪治疗腰椎间盘突出症可提高疗效,有助于缓解疼痛,改善腰椎功能,值得临床推广应用。

Objective To observe the efficacy of Tuina and traction combined with pulsation meter in the treatment of lumbar disc herniation.Methods Eighty patients with lumbar disc herniation admitted to our hospital from October 2017 to July 2020 were included in the study,and they were divided into observation group and control group by random number table method,with 40 cases in each group.The control group was given muscle relaxant and non-steroidal anti-inflammatory drugs,and the observation group was supplemented with Tuina and traction combined with pulsation meter.The total clinical effective rate,and scores of lumbar pain symptoms[visual analogue scale(VAS),Japan orthopedic association(JOA)],flexion range of motion of lumbar joint and lumbar function[functional independence measure(FIM)]before treatment and after 2 weeks of treatment were compared between the two groups.Results After 2 weeks of treatment,the total effective rate of treatment in observation group was significantly higher than that in the control group(P<0.05).The VAS scores of the two groups were lower than those before treatment,and the score in the observation group was lower than that in the control group,and the JOA scores were all higher than those before treatment,and the score in the observation group was higher than that in the control group(all P<0.05).The flexion ranges of motions of lumbar joint of the two groups were higher than those before treatment,and the range of motion in the observation group was higher than that in the control group(all P<0.05).The scores of sphincter control,self-care activities,progress and transfer of FIM scale in the two groups were all higher than those before treatment,and the scores in the observation group were higher than those in the control group(all P<0.05),but there were no statistically significant differences in the scores of communication and social cognition compared with those before treatment(all P>0.05).Conclusion Tuina and traction combined with pulsation meter can improve the efficacy of lumbar disc herniation,help to relieve pain and improve lumbar function,and thus it is worthy of clinical promotion and application.
