针灸联合穴位贴敷治疗前列腺增生电切术后尿潴留的临床研究

Clinical Study of Acupuncture Combined with Acupoint Application in Treatment of Urinary Retention after Electrosurgical Resection of Prostatic Hyperplasia

目的探讨针灸联合穴位贴敷治疗前列腺增生电切术后尿潴留的临床效果。方法选择2018年8月—2019年11月于该院行经尿道前列腺电切术且术后发生尿潴留的96例患者作为研究对象,以双盲法将其分为两组,其中对照组与治疗组各48例。对照组留置尿管定期开放、口服盐酸坦索罗辛缓释胶囊治疗,治疗组在此基础上应用针灸联合穴位贴敷治疗。对比两组临床疗效,以及治疗前与治疗后(2个疗程后)的国际前列腺症状评分(IPSS)、生活质量评分(QOL)、最大尿流率(Qmax)与残余尿量(RUV)。结果治疗组治疗的总效率为91.67%(44/48),对照组为75.00%(36/48),治疗组治疗的总有效率高于对照组,差异有统计学意义(P<0.05)。治疗后,两组IPSS评分与QOL评分均低于治疗前(P<0.05),其中治疗组IPSS评分与QOL评分均低于对照组(P<0.01)。治疗后,两组Qmax指标高于治疗前(P<0.05),RUV指标低于治疗前(P<0.01);其中治疗组Qmax指标高于对照组(P<0.01),RUV指标低于对照组(P<0.01)。结论针灸联合穴位贴敷治疗前列腺增生电切术后尿潴留疗效可靠,且使用安全,操作简便,值得临床推广。

Objective To investigate the clinical effect of acupuncture combined with acupoint application in the treatment of urinary retention after electrosurgical resection of prostatic hyperplasia.Methods From August 2018 to November 2019,96 patients who underwent ureteral prostatectomy in our hospital and experienced postoperative urinary retention were selected as the study objects.They were divided into two groups by double-blind method,including 48 cases in the control group and 48 cases in the study group.The indwelling catheter of the control group was opened regularly and the sustained release capsules of tamsolosin hydrochloride were taken orally.The clinical efficacy of the two groups as well as the international prostate symptom score(IPSS),quality of life score(QOL),maximum urine flow rate(Qmax)and residual urine volume(RUV)before and after treatment(2 courses)were compared.Results The total therapeutic efficiency of the study group was 91.67%(44/48)and that of the control group was 75.00%(36/48).The total therapeutic efficiency of the study group was higher than that of the control group,with statistically significant difference(P<0.05).After treatment,the IPSS score and QOL score of the two groups were lower than those before treatment(P<0.05),and the IPSS score and QOL score of the study group were lower than those of the control group(P<0.01).After treatment,the Qmax index of the two groups was higher than that before treatment(P<0.05),and the RUV index was lower than that before treatment(P<0.01).The Qmax index of the study group was higher than that of the control group(P<0.01),and the RUV index was lower than that of the control group(P<0.01).Conclusion The effect of acupuncture and acupoint application in the treatment of urinary retention after electrosurgical resection of prostatic hyperplasia is reliable,safe,simple and worthy of clinical promotion.
