冠心病患者中医心理紊乱状态及其相关因素的探讨

Discussion on Psychological Disorder and Related Factors in Patients with Coronary Heart Disease in Traditional Chinese Medicine

目的探究冠心病患者中医心理紊乱状态及其相关因素。方法选取2017年8月—2018年12月本院收治的冠心病患者300例,根据患者心理紊乱状态将其分为正常组(n=133)和心理紊乱组(n=167)。采用齐向华教授制订的中医心理紊乱状态量化表和相关因素调查表对300例冠心病的中医及相关因素心理紊乱状态进行分析。结果此300例冠心病患者共有167例(55.67%)存在不同程度的中医心理紊乱状态,思虑过度状态患者53例(31.74%),其次为烦躁焦虑状态45例(26.95%),郁闷不舒状态31例(18.56%),精神萎靡状态26例(15.57%),惊悸不安状态12例(7.19%);经多因素Logistic回归分析发现,患者年龄≥65岁、有高血压史、对疾病认知程度高以及病程长为冠心病患者合并中医心理紊乱状态的独立危险因素(P<0.05)。结论冠心病患者普遍存在一定的中医心理紊乱状态,分析患者的心理状态及相关因素,并作出积极有效的应对措施,对冠心病患者的预后有积极作用。

Objective To explore psychological disorders and related factors in patients with coronary heart disease in traditional Chinese medicine.Methods A total of 300 patients with coronary heart disease(CHD)admitted to our hospital from August 2017 to December 2018 were divided into normal group(n=133)and mental disorder group(n=167)according to their mental disorder status.The mental disorder status of 300 patients with CHD and related factors were analyzed by using the quantitative table of TCM mental disorder and related factors questionnaire developed by Professor QI Xianghua.Results A total of 167 patients(55.67%)with CHD had different degrees of TCM mental disorder,53 patients(31.74%)with over-thinking state,45 patients(26.95%)with agitation,31 patients(18.56%)with depression,26 patients(15.57%)with depression,and 12 patients(7.19%)with panic and restlessness.The independent risk factors of CHD patients with TCM mental disorder were age(>65 years old),history of hypertension,high awareness of disease and long course of disease(P<0.05).Conclusion There are some psychological disorders in traditional Chinese medicine in patients with coronary heart disease.The analysis of the patients′psychological status and related factors and the positive and effective countermeasures have a positive effect on the prognosis of patients with coronary heart disease.
