补肾温阳止痛汤联合推拿治疗腰椎骨质增生的疗效及对患者中医证候、腰功能及生活质量的影响

Efficacy of Bushen Wenyang Zhitong Decoction Combined with Tuina on Lumbar Vertebrae Hyperosteogeny and Its Influence on TCM Symptoms,Lumbar Function and Quality of Life

目的观察补肾温阳止痛汤与推拿联合治疗腰椎骨质增生的效果及对患者中医证候、腰功能、生活质量等影响。方法收集2015年7月1日—2018年6月30日本院收治的98例腰椎骨质增生患者,按就诊号奇偶数法分为观察组与对照组各49例,两组均有常规西药治疗、牵引治疗,此外对照组给予推拿手法治疗,观察组对照组之上给予补肾温阳止痛汤,连续治疗4周后,比较两组中医证候积分及证候疗效、X线检查评分、疼痛[视觉模拟评分(VAS)]、腰功能评分[Oswestry功能障碍指数问卷(ODI)]及生活质量[生活质量核心量表(QOL-30)]。结果治疗后,观察组中医证候积分显著低于对照组(P<0.05);观察组证候疗效93.88%(46/49),显著高于对照组的77.55%(38/49)(P<0.05);观察组X线检查评分、VAS评分、ODI评分均显著低于对照组(P<0.05),QOL-30评分中躯体、情绪、社会及生活总质量维度评分显著高于对照组(P<0.05)。结论补肾温阳止痛汤配合推拿手法用于腰椎骨质增生治疗,可以有效缓解患者中医证候,抑制骨质增生,有利于减轻患者疼痛、促进腰功能恢复并提高患者生活质量。

Objective To observe the effects of Bushen Wenyang Zhitong Decoction combined with Tuina on lumbar vertebrae hyperosteogeny and its influence on TCM symptoms,lumbar function and quality of life.Methods Ninety-eight cases of patients with lumbar vertebrae hyperosteogeny admitted to the hospital from July 1,2015 to June 30,2018 were collected and divided into observation group and control group according to the odd and even visit numbers,with 49 cases in each group.The two groups were treated with conventional western medicine and traction therapy,and control group was treated with Tuina manipulation,and observation group was given Bushen Wenyang Zhitong Decoction on the basis of the control group.After 4 weeks continuous treatment,the TCM syndromes scores and symptom efficacy,X-ray examination score,pain[visual analogue scale(VAS)]and lumbar function score[Oswestry dysfunction index(ODI)]and quality of life[quality of life core scale(QOL-30)]were compared between the two groups.Results After treatment,the TCM syndromes scores in the observation group were significantly lower than those in the control group(P<0.05).The symptom efficacy in observation group was significantly higher than that in the control group[93.88%(46/49)vs 77.55%(38/49)](P<0.05).The scores of X-ray examination,VAS and ODI in observation group were significantly lower than those in the control group(P<0.05),and the scores of dimensions of body,emotion and society and total quality of life of QOL-30 were significantly higher than those in the control group(P<0.05).Conclusion Bushen Wenyang Zhitong Decoction combined with Tuina manipulation for lumbar vertebrae hyperosteogeny can effectively alleviate TCM symptoms and inhibit hyperosteogeny,and help to relieve pain,promote lumbar function recovery and enhance quality of life.
