喉罩通气麻醉对卵巢囊肿患者术中血流动力学的影响

Effects of Laryngeal Mask Ventilation Anesthesia on Intraoperative Hemodynamics in Patients with Ovarian Cysts

目的:分析喉罩通气麻醉对卵巢囊肿患者术中血流动力学的影响。方法:选取2018年1月~2020年4月间我院接收的116例卵巢囊肿患者,均行腹腔镜卵巢囊肿剔除术治疗,依据随机数字表法分成喉罩组与气管组,各58例。喉罩组行喉罩通气麻醉,气管组行气管插管全麻,对比两组麻醉诱导前(T0)、带喉罩或插管时(T1)、拔除喉罩或拔管时(T2)血流动力学指标[平均动脉压(MAP)、心率(HR)]、应激反应[皮质醇(Cor)、肾上腺素(E)、去甲肾上腺素(NE)]以及并发症发生率。结果:T1、T2时喉罩组MAP、HR小于气管组(P<0.05);T1、T2时喉罩组Cor、NE、E低于气管组(P<0.05);与气管组17.24%(10/58)对比,喉罩组并发症发生率5.17%(3/58)明显降低(P<0.05)。结论:喉罩通气麻醉应用于卵巢囊肿术中,可减轻应激反应,维持血流动力学稳定,减少并发症发生。

Objective:To analyze the effect of laryngeal mask ventilation anesthesia on intraoperative hemodynamics in patients with ovarian cysts.Methods:A total of 116 patients with ovarian cysts received in our hospital from January 2018 to April 2020 were selected for laparoscopic ovarian cystectomy.According to the random number table method,they were divided into laryngeal mask group and trachea group,58 cases in each.The laryngeal mask group underwent laryngeal mask ventilation anesthesia,and the trachea group underwent tracheal intubation for general anesthesia.Comparing the hemodynamic indexes[mean arterial pressure(MAP),heart rate(HR)],stress responses[cortisol(Cor),epinephrine(E),norepinephrine(NE)]before anesthesia induction(T0),with laryngeal mask or intubation(T1),when removing the laryngeal mask or extubation(T2)and the incidence of complications of the two groups.Results:The MAP and HR of the laryngeal mask group at T1 and T2 were lower than those of the trachea group(P<0.05);Cor,NE,E in laryngeal mask group were lower than tracheal group at T1 and T2(P<0.05);compared with17.24%(10/58)of the trachea group,the incidence of complications in the laryngeal mask group was 5.17%(3/58),which was significantly reduced(P<0.05).Conclusion:The application of laryngeal mask ventilation anesthesia in ovarian cyst surgery can reduce stress response,maintain hemodynamic stability and reduce complications.
