冠心病患者超声心动图改变与心功能及冠脉狭窄程度的相关性

Correlation analysis of echocardiographic indicators with cardiac function and coronary stenosis in patients with coronary heart disease

目的分析冠心病患者超声心动图指标与其心功能、冠脉狭窄之间的相关性。方法选2020年1月至2022年12月江西省胸科医院收治的80例冠心病患者为研究组,同期体检100名健康对象为对照组。两组均行超声心动图检查[左室射血分数(LVEF)、左室舒张末期容积(LVEDV)、左室收缩末期容积(LVESV)],对不同心功能等级、不同冠脉狭窄程度冠心病患者的超声心动图检查结果行比较。结果研究组的LVEF低于对照组,LVEDV、LVESV高于对照组,差异有统计学意义(P<0.05);心功能等级为Ⅳ级的冠心病患者LVEF低于Ⅲ级、Ⅱ级、Ⅰ级冠心病患者,同时LVEDV、LVESV高于Ⅲ级、Ⅱ级、Ⅰ级冠心病患者,差异有统计学意义(P<0.05);重度冠脉狭窄冠心病患者的LVEF低于中度、轻度患者,同时LVEDV、LVESV高于中度、轻度患者,差异有统计学意义(P<0.05);冠心病患者超声心动图检测结果中LVEF与心功能等级、冠脉狭窄程度呈负相关性(P<0.05),而LVEDV、LVESV与心功能等级、冠脉狭窄程度呈正相关性(P<0.05)。结论冠心病患者超声心动图指标中的LVEDV、LVESV与其心功能及冠脉狭窄呈正相关性,而LVEF与其心功能及冠脉狭窄呈负相关性,对判断冠心病患者病情发展有积极作用。

Objective To analyze the correlation between echocardiographic indicators and cardiac function and coronary artery stenosis in patients with coronary heart disease.Methods Eighty patients with coronary heart disease admitted to Jiangxi Chest Hospital from January 2020 to December 2022 were selected as the study group,and 100 healthy subjects were selected as the control group during the same period of physical examination.Both groups were examined by echocardiography[left ventricular Ejection fraction(LVEF),left ventricular end diastolic volume(LVEDV),left ventricular end systolic volume(LVESV)],and the results of echocardiography in patients with coronary heart disease with different levels of concentric function and different degrees of coronary stenosis were compared.Results The LVEF of the study group was lower than that of the control group,while LVEDV and LVESV were higher than those of the control group,with a statistically significant difference(P<0.05).LVEF of coronary heart disease patients with a heart function grade ofⅣwas lower than that of patients with gradeⅢ,Ⅱ,and I coronary heart disease,while LVEDV and LVESV were higher than those of gradeⅢ,Ⅱ,and I coronary heart disease patients,with a statistically significant difference(P<0.05).The LVEF of patients with severe coronary artery stenosis and coronary heart disease was lower than that of moderate and mild patients,while LVEDV and LVESV were higher than those of moderate and mild patients,with a statistically significant difference(P<0.05).The echocardiographic examination results of coronary heart disease patients showed a negative correlation(P<0.05)between LVEF and cardiac function grade and degree of coronary stenosis,while LVEDV and LVESV showed a positive correlation(P<0.05)with cardiac function grade and degree of coronary stenosis.Conclusions LVEDV and LVESV in echocardiographic indicators of coronary heart disease patients are positively correlated with their cardiac function and coronary stenosis,while LVEF is negatively correlated with their cardiac function and coronary stenosis,which has a positive role in judging the development of coronary heart disease patients.
