沙库巴曲缬沙坦联合芪参益气滴丸治疗慢性心力衰竭

Sacubitril valsartan combined with Qishen Yiqi dropping pills for chronic heart failure

目的探讨联合应用沙库巴曲缬沙坦钠片和芪参益气滴丸治疗慢性心力衰竭的临床疗效;对于慢性心力衰竭患者的心功能、生存质量与心衰生物标志物的影响。方法选取2020年4月至6月在芜湖市中医医院心血管内科就诊的慢性心力衰竭患者共计60例作为研究对象,随机分为对照组与观察组,对照组30例,观察组30例。其中对照组予以沙库巴曲缬沙坦钠片治疗,观察组予以联合应用沙库巴曲缬沙坦钠片、芪参益气滴丸治疗,共治疗3个月,观察两组临床疗效,明尼苏达心力衰竭生存质量量表(MLHFQ)测评,心衰生物标志物[N末端B型利钠肽前体(NT-proBNP)、肿瘤坏死因子-α(TNF-α)、白介素(IL)-1β、IL-6],心功能指标[左室舒张末期容积(LVEDV)、左室收缩末期容积(LVESV)、左室射血分数(LVEF)]变化。结果观察组治疗3个月后临床总有效率(96.67%)较对照组(73.35%)高(P<0.05)。对照组与观察组治疗90天后MLHFQ评分均较治疗前降低(P<0.05),观察组疗程结束后MLHFQ评分明显低于观察组(P<0.05)。观察组治疗90天后NT-proBNP、TNF-α、IL-1β、IL-6均较对照组低(P<0.05)。治疗90天后对照组与观察组LVEDV、LVESV较同组治疗前减少(P<0.05),LVEF较同组治疗前提高(P<0.05);两组间比较差异无统计学意义(P>0.05)。结论联合应用沙库巴曲缬沙坦钠片、芪参益气滴丸治疗慢性心力衰竭疗效确切,能够更好地改善心功能与生存质量。

Objective To investigate the clinical effect of sacubitril valsartan sodium tablets and Qishenyiqi dropping pills in the treatment of chronic heart failure.Effects of heart function,quality of life and heart failure biomarkers in patients with chronic heart failure.Methods A total of 60 patients with chronic heart failure who were treated in the Department of Cardiovascular Medicine,Wuhu Hospital of Traditional Chinese Medicine from April 2020 to June 2022 were selected as the research objects.They were randomly divided into control group(n=30)and observation group(n=30).The control group was treated with sacubitril valsartan sodium tablets,while the observation group was treated with sacubitril valsartan sodium tablets and Qishen Yiqi dropping pills for a total of three months.The clinical efficacy of the two groups was observed.MLHFQ assessment,heart failure biomarkers(NT-proBNP,TNF-α,IL-1β,IL-6),changes of cardiac function indexes(LVEDV,LVESV,LVEF)were observed.Results After three months of treatment,the total effective rate of observation group(96.67%)was higher than that of control group(73.35%)(P<0.05).After 90 days of treatment,the MLHFQ scores of the control group and the observation group were lower than those before treatment(P<0.05),and the MLHFQ scores of the observation group were significantly lower than those of the observation group after treatment(P<0.05).After 90 days of treatment,NT-proBNP,TNF-α,IL-1βand IL-6 in the observation group were lower than those in the control group(P<0.05).After 90 days of treatment,the LVEDV and LVESV of the control group and the observation group were decreased compared with those before treatment(P<0.05),and the LVEF was increased compared with those before treatment(P<0.05).There was no significant difference between the two groups(P>0.05).Conclusions Combined application of sacubitril valsartan and Qishenyiqi dropping pills in the treatment of chronic heart failure is effective and can improve heart function and quality of life.
