基于PDCA循环法的含毒性成分中成药的合理用药管理模式构建与实施

Construction and implementation of the rational medication management model for Chinese patent medicines containing toxic components based on the PDCA cycle method

摘要 目的使用PDCA循环法构建含毒性成分中成药的合理用药管理模式,提高该类中成药的合理用药水平。方法采用PDCA循环法,回顾性分析2021至2022年我院含毒性成分中成药不合理用药中涉及的人员、信息、制度、环境等方面的原因,再根据其主要原因制定对策并实施,对比管理模式实施前后含毒性成分中成药不合理用药处方比例及不良反应发生率。结果实施PDCA循环法管理后,处方不合理率由10.25%降低至2.99%(P<0.01),适应证不适宜、用法用量不适宜、重复用药、配伍禁忌等不合理问题均得到有效控制(P<0.05),且不良反应发生率也显著降低(P<0.05)。结论基于PDCA循环法的含毒性成分中成药的合理用药管理模式可有效降低含毒性成分中成药的不合理处方比例,促进合理用药,保障用药安全。 AIM To establish a rational medication management model for Chinese patent medicines containing toxic components by the application of PDCA cycle method and to improve the rational use of these medicines.METHODS The PDCA cycle method was used to retrospectively analyze the reasons of personnel,information,system,environment and other aspects involved in the irrational use of Chinese patent medicines containing toxic components in our hospital from 2021 to 2022.Countermeasures were formulated and implemented based on the main reasons.The proportion of irrational prescriptions and the incidence of adverse reactions for Chinese patent medicines containing toxic components before and after implementation were compared.RESULTS Following the implementation of the PDCA cycle method,the rate of irrational prescriptions significantly decreased from 10.25%to 2.99%(P<0.01).Issues such as inappropriate indications,improper dosage and administration,redundant medication,and contraindicated combinations were effectively controlled(P<0.05).The incidence of adverse reactions also significantly reduced.CONCLUSION The PDCA cycle method for Chinese patent medicines containing toxic components can effectively reduce the proportion of irrational prescriptions,promote rational drug use,and ensure medication safety.
