基于AHM赋权法和TOPSIS法的转化糖电解质注射液合理性评价

Evaluation on rationality of invert sugar and electrolyte injection based on AHM weighting method and TOPSIS method

目的 运用属性层次分析模型赋权法(AHM)和逼近理想解排序法(TOPSIS),对转化糖电解质注射液的合理使用进行评价、分析,为临床合理用药提供参考。方法 以《转化糖电解质注射液说明书》《医院处方点评管理规范(试行)》《医疗机构药事管理规定》及相关文献为基础,制定转化糖电解质注射液合理使用评价标准。采用AHM赋权法计算评价指标相对权重,采用TOPSIS法对536例使用转化糖电解质注射液的病历进行综合分析。结果 536例病历中相对接近度≥60%的有509例(占94.96%),50%~60%的有26例(占4.85%),30%~40%的有1例(占0.19%)。判定为合理用药的病历共301份(占56.16%),基本合理用药的共208份(占38.81%),不合理用药的共27份(占5.04%)。本次转化糖电解质注射液的处方点评共涉及2个不同厂家,2种不同规格。结论 与传统方法比,AHM赋权法和TOPSIS法结合多个因素对转化糖电解质注射液的合理性进行评价,其结果更直观、可靠。临床对转化糖电解质注射液使用应该规范其适应证、给药疗程等,进一步促进合理使用。

AIM To evaluate and analyze the rational use of invert sugar and electrolyte injection by using attibute hierarchical mode(AHM)weighting method and technique for order preference by similarity to an ideal solution(TOPSIS)method,and to provide reference for rational drug use in clinic.METHODS Based on Invert sugar electrolyte injection instrucoion,Management specification for hospital prescription review(Trial),Regulations on pharmaceutical administration of medical institutions and related literatures,the evaluation criteria for rational use of invert sugar and electrolyte injection was established.The AHM weighting method was used for calculating the relative weights of evaluation indicators.TOPSIS method was used for comprehensively analyzing the medical records of 536 cases of invert sugar and electrolyte injection.RESULTS Among the 536 medical records,509 cases(94.96%)had relative proximity≥60%,26 cases(4.85%)of 50%-60%,and 1 case(0.19%)of 30%-40%.A total of 301 cases(56.16%)were judged to be rational drug use,208 cases(38.81%)were basically rational drug use,and 27 cases(5.04%)were irrational drug use.The prescription review of invert sugar and electrolyte injection was involved two different manufacturers and two specifications.CONCLUSION Compared with traditional methods,AHM weighting method and TOPSIS method combined multiple factors can be used to evaluate the rationality of invert sugar and electrolyte injection,and the results are more intuitive and reliable.The indications and course of administration of invert sugar and electrolyte injection should be standardized to further promote rational use.
