冠状动脉痉挛致急性心肌梗死及恶性心律失常猝死1例

Acute myocardial infarction and sudden death because of malignant arrhythmia caused by coronary artery spasm:one case report

1病例资料患者,男,55岁,因阵发性胸闷7年,加重12 h急诊入院。患者7年前常于夜间睡眠时出现阵发性胸骨后憋闷感及心前区紧缩性疼痛,含服"速效救心丸"10~20 min可缓解。1个月前因上述症状再发就诊于我院,心电图示:V2-V5导联T波倒置(图1),肌钙蛋白I 0.07 ng/ml,冠状动脉(冠脉)造影示右冠脉近段少许斑块影(图2),诊断为"冠脉粥样硬化;高血压病2级(高危组)"。

This paper reports a case of acute myocardial infarction and sudden death because of malignant arrhythmia caused by different coronary arteries spasm.Based on related domestic and foreign literatures,this paper discusses the causes of coronary artery spasm and the different clinical manifestations,so as to improve the recognition and intervention of this disease by clinicians and strengthen the health education of patients in order to reduce the occurrence of cardiovascular events caused by coronary artery spasm.
