腔内介入治疗锁骨下动脉硬化性狭窄/闭塞的疗效分析

Analysis of the Effect of Endovascular Therapy on Subclavian Arteriosclerotic Stenosis/Occlusion

目的评估腔内介入治疗对锁骨下动脉硬化性狭窄/闭塞的临床治疗效果。方法搜集2013年1月至2019年6月于本院的锁骨下动脉硬化性狭窄/闭塞患者的临床病历资料,并进行综合分析总结。结果符合入选条件的患者共47例,其中股动脉入路38例,肱动脉入路2例,联合入路7例,技术成功率100%,术后所有患者症状均得到不同程度的改善,术前、术后双上臂收缩压差分别为(37.36±6.38)mmHg、(12.11±3.72)mmHg,具有统计学意义(P<0.05);30天内无患者死亡及脑卒中发生;术后6个月、1年、2年、3年、4年、5年的初级通畅率分别为98.45%、92.38%、88.61%、83.47%、75.25%、70.33%。结论腔内介入治疗对锁骨下动脉硬化性狭窄/闭塞是目前比较安全、有效的治疗方式,并且其中远期通畅率也得到证实。

Objective To evaluate the clinical effect of endovascular interventional therapy on subclavian artery stenosis/occlusion.Methods Clinical medical records of patients with subclavian arteriosclerotic stenosis/occlusion hospitalized in our department from January 2013 to June 2019 were collected,and a comprehensive analysis and summary was conducted.Results A total of 47 patients met the enrollment criteria,and all patients had improved symptoms to varying degrees after surgery,including 38 femoral artery access,2 brachial artery access,and 7 combined approach.The technical success rate was 100%.The systolic pressure difference between the arms before and after the operation was(37.36±6.38)mmHg and(12.11±3.72)mmHg,respectively,with statistical significance(P<0.05).No patient death or stroke occurred within 30 days.The The primary patency rates were 98.45%,92.38%,88.61%,83.47%,75.25%and 70.33%at 6-month,1-,2-,3-,4-,and 5-year after surgery.Conclusion Endovascular interventional therapy is a safe and effective treatment for subclavian artery stenosis/occlusion,and there is confirmed high long-term patency rate.
