摘要
目的采用Meta分析评价酰胺质子转移(APT)MRI对低级别和高级别胶质瘤的鉴别诊断价值。方法全面检索PubMed、Ovid、EBSCO、EMbase、CNKI、万方数据库、CBM和维普数据库,纳入鉴别低级别和高级别胶质瘤的APT-MRI文献。采用Stata 13.0软件计算合并敏感度、特异度、诊断比值比(DOR)、阳性似然比(LR+)、阴性似然比(LR-),并绘制汇总受试者工作特征曲线(SROC)和计算曲线下面积(AUC)。结果共纳入10篇文献(包括353例研究对象)。合并敏感度、特异度、DOR、LR+和LR-分别为88%(95%CI 77%~94%)、91%(95%CI 82%~96%)、73(95%CI 24~222)、9.5(95%CI 4.6~19)和0.13(95%CI 0.06~0.26),AUC为0.95(95%CI 0.93~0.97)。结论APT-MRI对低级别和高级别胶质瘤具有较高的鉴别诊断价值,可作为临床胶质瘤分级的可靠方法之一。
Objective To evaluate the diagnostic value of amide proton transfer(APT)MRI for the differential diagnosis of low and high grade gliomas with a meta analysis.Methods Publications from the PubMed,Ovid,EBSCO,EMbase,CNKI,WanFang,CBM and VIP databases were comprehensively searched,and the papers that evaluating APT-MRI for the differential diagnosis of low and high grade gliomas were included.The pooled Sen,Spe,DOR,LR+and LR-were calculated,and the SROC curve was constructed with Stata 13.0 software.Results 10 papers with 353 subjects were finally included.The pooled Sen,Spe,DOR,LR+and LR-were 88%(95%CI,77-94%),91%(95%CI,82-96%),73(95%CI,24-222),9.5(95%CI,4.6-19)and 0.13(95%CI,0.06-0.26),respectively,and the area under the SROC curve was 0.95(95%CI,0.93-0.97).Conclusion APT-MRI showed good diagnostic value for differentiating low and high grade gliomas.Thus,APT-MRI could be one of reliable modalities for glioma grading in clinical practice.
