机器学习法在HIV感染者功能磁共振成像中DC值的研究 被引量:3

Study on DC Value of Machine Learning in Functional Magnetic Resonance Imaging of AIDS Patients

摘要 目的运用基于机器学习法的静息态功能磁共振成像(rs-fMRI)技术,探究度中心性(DC值)在HIV相关神经认知障碍(HAND)中的诊断价值。方法搜集未进行抗病毒治疗的HIV感染者27例(HIV组)和正常志愿者14名(对照组)。以神经心理测评(NP)测验为参考标准,将HIV组分为HAND组和非HAND组。经磁共振扫描后所得图像使用DPARSF和PRoNTo软件行后处理。结果DC指标在HIV组与对照组差异贡献最大的十个大脑区域依次是右旁中央小叶、左旁中央小叶、右枕上回、右辅助运动区、右楔叶、左楔叶、左顶上回、左楔前叶、左辅助运动区、右中央后回。DC指标对区别HAND组及非HAND组有意义。左楔前叶、楔叶、右枕上回、右辅助运动区的DC值与部分临床血液学、临床量表、认知评分及认知分组有不同程度的相关性。结论DC值为临床诊断HAND提供客观的标准,对降低HAND的发生和发展可起一定的作用。 Objective To explore the diagnostic value of DC values in the assessment of HIV-related neurocognitive disorders using magnetic resonance resting state functional imaging based on machine learning.Methods 27 AIDS patients without antiviral therapy(HIV group)and 14 normal volunteers(control group)were included.Taking the NP(neuropsychological performance test)as reference standard,the HIV patients were grouped into the HAND(HIV-associated neurocognitive disorder)and the non-HAND settings.The images from magnetic resonance scanning were post-processed using DPARSF and PRoNTo software.Results The ten brain regions in which the DC index contributed the most difference between the AIDS patients and the control group included the right paracentral lobule,the left paracentral lobule,the right superior occipital gyrus,the right supplementary motor area,the right cuneus,the left cuneus,the left superior parietal gyrus,the left precuneus,the left supplementary motor area,and the right postcentral gyrus.DC index was significant between HAND group and non-HAND group.The DC values of the left paracentral lobule,the left cuneus,the right superior occipital gyrus and the right supplementary motor area were related to hematology,clinical scales,cognitive scores,MMSE grouping and NP groupings.Conclusion DC value is a useful marker for HAND,which may contribute to the occurrence and development of HAND.
