摘要
目的探讨腰椎间盘突出中扩散张量成像(DTI)参数各向异性分数(FA)值、表观扩散系数(ADC)值、本征向量值(λ//和λ⊥)的改变,分析FA值、ADC值、λ//、λ⊥在腰椎间盘突出的作用。方法搜集临床诊断及MRI平扫显示为单侧S1神经根受压的40例腰椎间盘突出症患者,进行DTI检查,测量FA值、ADC值、λ//和λ⊥。结果受压侧各段FA值具有明显差异(F=4.852,P=0.021),其中受压段与受压近段具有显著差异(P<0.05),受压段与远段无差异;受压侧各段λ⊥值有显著性差异(F=5.135,P=0.017),其中受压段与受压近段(P<0.05)、受压远段(P<0.05)均具有差异;ADC与λ//无明显差异;受压侧神经根FA值、λ⊥与健侧对应神经根具有明显差异性(t=-2.867,P=0.012;t=3.556,P=0.003);ADC与λ//无明显差异。结论DTI作为一种功能成像,能从宏观及微观角度评价腰椎间盘突出致骶神经根病变。
Objective To investigate the changes of FA value,ADC value,λ_//,λ_⊥,Diffusion Tensor Imaging(DTI)parameters in lumbar intervertebral disc herniation,and to obtain the effects of FA value,ADC value,λ_//andλ_⊥on lumbar intervertebral disc herniation.Methods 40 cases who unilateral compression of S1 nerve root were selected with lumbar intervertebral disc herniation.MRI DTI scan was performed with all cases,obtained FA value,ADC value,λ_//andλ_⊥.Results In the compression side of S1 nerve root,the FA values of each injury portion were significantly different(F=4.852,P=0.021),the compressed portion had significantly difference with the proximal portion(P<0.05),and not difference with the distal portion.Theλ_⊥had significant difference(F=5.135,P=0.017)in the compressed side,include theλ_⊥in the compressed portion was significantly higher than proximal(P<0.05)and distal portion(P<0.05).There was no significant difference between ADC andλ_∥.The FA values andλ_⊥were significantly different between the compressed and contralateral nerve roots(t=-2.867,P=0.012,t=3.556,P=0.003,).There was no significant difference in ADC andλ_∥.Conclusion Diffusion tensor imaging(DTI),as a functional imaging,can evaluate the sacral nerve root injury caused by lumbar disc herniation from from a macroscopic and microscopic perspective.
