基于阴阳辨证分析MSA-C型患者VNG及疾病变化特点 被引量:2

Yin and Yang syndrome differentiation-based videonystagmograph and disease change characteristics in patients with MSA-C

摘要 目的基于阴阳辨证分析多系统萎缩C型(MSA-C型)患者视频眼震电图(VNG)及疾病变化特点。方法收集本院神经内科确诊MSA-C型患者25例,完善VNG相关检查及MSA评价量表等信息采集,并按照中医辨证分型标准由中医师进行阳虚、阴虚分组,对比分析眼震电图特点及与病变程度、中医证候之间的相关性,总结疾病发展变化规律。结果MSA-C型患者眼震电图以凝视试验、扫视试验、视跟踪眼动反应等中枢性眼动异常表现为主,且以阴虚为主者在眼动功能障碍程度、病程及病变致残程度上重于阳虚组。结论随着疾病发展,MSA-C型患者中医证候变化有以阳虚为主到阴虚更甚的特点,且与病程、中枢功能损伤及病变程度、病变范围存在正相关,且除小脑损伤渐重之外,还存在其他中枢神经系统区域包括锥体系及皮层。 Objective Videonystagmographz(VNG)and disease change characteristics of Multiple System Atrophy-C(MSA-C)were analyzed based on Yin and Yang syndrome differentiation.Methods Twenty five cases with MSA-C confirmed by our hospital neurology department were collected.The VNG and MSA rating scale information acquisition were conducted,and the patients were divided into Yang deficiency or Yin deficiency groups in accordance with criteria of TCM syndrome differentiation by veteran traditional Chinese medicine doctors.Then the correlations of VNG characteristics with the degree of pathological changes,and with traditional Chinese medicine syndrome were compared so as to summarize the change law of disease development.Results VNG of patients with MSA-C was characterized by central eye movement abnormalities,including gaze test,saccasmus test,visual tracking eye movement response,and those with Yin deficiency were mainly manifested as more severe in the degree of eye movement dysfunction,course of disease and lesion disability than those with Yang deficiency.Conclusion For MSA-C who mainly focus on cerebellum injury,with the development of the disease and the continuation of the disease course,traditional Chinese medicine syndromes are positively correlated with the disease course,central function injury and the degree of lesion.In addition to cerebellum injury,there are also other central nervous system regions affected including the cone system and cortex involved.
