双气囊小肠镜对不同年龄患者小肠疾病诊断价值研究

The diagnostic value of double-balloon enteroscopy for intestinal diseases in patients of different ages

目的比较不同年龄段接受双气囊小肠镜(DBE)检查的患者镜下诊断的病变类型及安全性。方法选取北部战区总医院自2009年5月至2020年8月收治的707例接受DBE检查的患者为研究对象。按照年龄将其分为青年组(年龄≤40岁,n=276)与中老年组(年龄>40岁,n=431)。比较两组患者内镜下诊断病变类型及并发症发生情况。结果DBE检查对病变的总检出率为69.7%(493/707)。青年组患者息肉、黏膜隆起、炎症性肠病检出率均高于中老年组,胃及结肠病变、占位/肿瘤检出率均低于中老年组,差异均有统计学意义(P<0.05)。青年组、中老年组患者术后并发症发生率分别为0.7%(2/276)、0.5%(2/431),组间比较,差异无统计学意义(P>0.05)。结论DBE对小肠疾病具有较高的检出率,青年和中老年的疾病谱略有不同,DBE术后并发症较少,安全性良好。

Objective To compare the types and safety of the lesions diagnosed under the microscope in patients of different ages who underwent double-balloon enteroscopy(DBE).Methods A total of 707 DBE patients admitted to General Hospital of Northern Theater Command from May 2009 to August 2020 were selected as the study subjects.They were divided into young group(≤40 years old,n=276)and elder group(>40 years old,n=431)according to age.The types of lesions and complications diagnosed under endoscopy were compared between the two groups.Results The total detection rate of DBE was 69.7%(493/707).The detection rates of polyp,mucosal eminence and inflammatory bowel disease in the young group were higher than those in the elder group,while the detection rates of gastric and colon lesions and space occupation/tumor were lower than those in the elder group,with statistical significance(P<0.05).The incidence of postoperative complications was 0.7%(2/276)in the young group and 0.5%(2/431)in the elder group,respectively,with no statistical significance(P>0.05).Conclusion DBE has a high detection rate of small intestinal diseases,and the disease spectrum is slightly different between young and middle-aged people.DBE has fewer postoperative complications and good safety.
