高危型人乳头瘤病毒与液基薄层细胞学检查技术联合检测在宫颈癌前病变筛查中诊断价值

Diagnostic value of combined detection of human papilloma virus and thinprep cytologic test in screening of cervical precancerous lesions

目的探讨高危型人乳头瘤病毒(HPV)与液基薄层细胞学检查技术(TCT)联合检测在宫颈癌前病变筛查诊断中的作用。方法回顾性分析2019年1—12月于北部战区总医院妇科门诊同时进行HPV、TCT和病理检查的1224例患者的临床资料。观察患者的年龄分布,比较HPV、TCT单独检测及HPV与TCT联合检测的病理诊断符合率。结果患者年龄17~80岁,平均年龄(40.00±11.12)岁;患者主要分布在25~34岁以及35~44岁年龄段。各年龄组HPV低危型感染检出率检出率比较,差异无统计学意义(P>0.05)。各年龄组HPV高危型感染检出率检出率比较,差异有统计学意义(P<0.05)。HPV联合TCT检测的病理诊断符合率高于HPV及TCT单独检测,差异均有统计学意义(P<0.05)。结论HPV和TCT联合检测可有效提高宫颈癌前病变检出率,为临床决策提供科学参考。

Objective To investigate the role of high risk human papilloma virus(HPV)combined with liquid based thinprep cytologic test(TCT)in screening and diagnosis of cervical precancerous lesions.Methods A retrospective study was performed on the clinical data of 1,224 patients who underwent HPV,TCT and pathological examination simultaneously from January to December 2019.The age distribution of the patients was observed,and the pathological diagnostic coincidence rate of HPV and TCT tests alone and combined tests of HPV and TCT were compared.Results The patients were 17~80 years old,with an average age of(40.00±11.12)years old.The patients were mainly distributed in 25~34 years old and 35~44 years old.The detection rate of low-risk HPV infection in all age groups was compared,and the difference was not statistically significant(P>0.05).The detection rate of high-risk HPV infection in all age groups was compared,and the difference was statistically significant(P<0.05).The pathological diagnosis coincidence rate of HPV combined with TCT was higher than that of HPV and TCT alone,P<0.05.Conclusion The combined detection of HPV and TCT can effectively improve the detection rate of cervical precancerous lesions and provide scientific reference for clinical decision-making.
