分娩镇痛对产妇发热及新生儿评分影响

Effect of labor analgesia on maternal fever and neonatal score

目的探讨分娩镇痛对产妇发热及新生儿评分的影响。方法选取自2016年1月至2017年12月于北部战区总医院和平院区妇产科进行阴道分娩的4931例产妇为研究对象。按照是否接受分娩镇痛将产妇分为无痛分娩组(n=2184)与未镇痛组(n=2747)。无痛分娩组采用硬膜外阻滞分娩镇痛,未镇痛组未采用镇痛。比较两组产妇的发热、产后出血情况及新生儿Apgar评分情况。结果无痛分娩组产妇发热率为12.0%(263/2184),明显高于未镇痛组的2.4%(263/2747),差异有统计学意义(P<0.05)。两组产妇产后出血比例,以及新生儿Apgar评分情况比较,差异均无统计学意义(P>0.05)。结论顺产采取硬膜外分娩镇痛会增加产妇生产期间发热的发生率,但与产后出血无关,且对新生儿评分无影响。

Objective To investigate the effect of labor analgesia on maternal fever and neonatal score.Methods A retrospective study was performed on 4931 cases of parturient women who were admitted and had vaginal delivery from January 2016 to December 2017.According to whether delivery analgesia was required,the women were divided into the painless delivery group(n=2184)and the non-analgesic group(n=2747).Epidural analgesia was used in painless delivery group and no analgesia was used in non-analgesic group.The fever,postpartum hemorrhage and Apgar score were compared between the two groups.Results The fever rate in the painless delivery group was 12.0%(263/2184),which was significantly higher than that in the non-analgesic group(2.4%,263/2747),and the difference was statistically significant(P<0.05).There were no statistically significant differences between the two groups in the proportion of postpartum hemorrhage and the Apgar score of newborns(P>0.05).Conclusion Epidural analgesia during vaginal delivery increases the incidence of fever during childbirth,but has no effect on postpartum hemorrhage,and has no effect on neonatal score.
