国人左心室壁内的动脉构筑

THE ARTERIAL OF THE LEFT VENTRICULAR WALL IN CHINESE

作者对正常人心离体标本52例分别用铸型法、X线法和墨汁注射火棉胶切片法作左心室壁内及乳头肌的动脉构筑研究。按血管形态、分布的混合命名法,将左室壁内的动脉分为心外膜支、树枝状支、直支和乳头肌支四种。树枝状支除到达左室壁的部分或全层心肌外,也分支到达肉柱,并在心内膜下与直支吻合构成心内膜下丛。乳头肌支为直支的特殊类型,呈多种钩形和叉形等不同形态,也有的呈“S”形的弯曲行径。研究了乳头肌的动脉构筑发现每个乳头肌至少有2支以上的血管,未见有单干中央型的乳头肌支。血管在乳头肌内的配布呈多种方位的立体构筑。对前后乳头肌的形态、大小及其血供进行了对比,讨论了它们和乳头肌缺血病变的关系。对左室壁内和乳头肌内的小血管和毛细血管的形态与心肌的关系作了观察,并讨论了它们形态和功能的一致性。

The normal arterial pattern of the left ventricular wall.including the papillary muscles was studied in 52 human heart postmortem by coronary angiography.arterial cast and histological section.According to the shape and distribution,the arteries in the left ventricular wall may be classified into four types,i e the epicardial,the arborize,the straight and the papillary arteries.The arborize arteries supplied the whole myocardial wall including some trabe-cular carneae and also anastomose with the straight arteries forming the subendocardial plexus.The papillary arteries are particular type of the straight artery whieh have various shape,such as:hooked,bifurcated or tortuous“S”shaped.With the study of histological sections and arterial casts,it is found that there is no so called single central core artery and present at least 2 arteries in each papillary muscle.The architecture of the papillary arteries appeared varioust ypes.The morphology and the blood supply of the anterior and posterior papillary muscles in the left ventricular wall were studied.The functional,pathalogical and clinical signi-ficances of these arteries were discussed.
