补泄理肾汤联合百令胶囊治疗早期糖尿病肾病临床研究

Clinical Study on Buxue Lishen Decoction Combined with Bailing Capsule in Treatment of Early Diabetic Nephropathy

目的观察对早期糖尿病肾病患者采用补泄理肾汤联合百令胶囊联合治疗的临床效果。方法将我院诊治的早期糖尿病肾病患者106例。随机分为观察组与对照组各53例。对照组给予厄贝沙坦联合百令胶囊治疗,观察组给予补泄理肾汤联合百令胶囊治疗。分析两组患者治疗前后的血糖代谢指标:空腹血糖(FPG)、餐后2h血糖(2h PG)、糖化血红蛋白(HbA1C)/血压与血脂指标;肾功能指标:胱抑素C(CysC)、尿素氮(BUN)及血肌酐(SCr)、血清同型半胱氨酸(HCY);检测患者24h尿中尿微量白蛋白(MAU)、尿微量白蛋白排泄率(UAER)水平变化情况,并记录中西医的临床治疗效果。结果治疗2个月后,观察组的中医证候疗效与临床疗效明显高于对照组(P<0.05);两组FPG、2hPG、HbA1C、HCY、MAU、UAER、Cys C、BUN、SCr、血压血脂水平(HDL-C除外)水平均明显降低,HDL-C水平上升,且观察组FPG、2hPG、HbA1C、HCY、MAU、UAER、CysC、BUN、SCr水平明显低于对照组(P<0.05),两组的血压血脂水平比较无统计学差异(P>0.05)。结论补泄理肾汤联合百令胶囊不仅可以降低早期糖尿病肾病患者血糖水平,还可以改善患者的血压、血脂与肾功能,降低HCY、MAU、UAER水平,临床疗效明显。

Objective To observe the clinical effect of Buxue Lishen Decoction combined with Bailing Capsule in treatment of early diabetic nephropathy.Methods 106 cases of patients with early diabetic nephropathy were randomly divided into two groups,53 cases in each group.Control group was treated with irbesatan combined Bailing Capsule,observation group was given Buliu Lishen Decoction on the basis of control group.Blood glucose metabolism indicators before and after treatment in both groups[fasting blood glucose(FPG),postprandial 2 h blood glucose(2 hPG),glycosylated hemoglobin(HbA1 C)],blood pressure,blood lipids,renal function indicators[cysin C(Cys C),urea nitrogen(BUN)and serum creatinine(SCr)],serum homocysteine(HCY),suggesting that the changes in urine microalbumin(MAU)and urinary albumin excretion rate(UAER)levels in 24 h were analyzed and the clinical treatment effect of Chinese and western medicine were recorded.Results After 2 months of treatment,the efficacy and clinical efficacy of TCM syndromes in observation group were were significantly higher than that in control group(P<0.05);after 2 months of treatment,the levels of the FPG 2 hPG,HbA1 C,HCY,MAU,UAER,Cys C,BUN,SCr,blood pressure and blood lipids(except HDL-C)of the two groups were significantly lower,the level of HDL-C increased,and the levels of FPG 2 hPG,HPbA1 C,HCY,MAU,UAER,Cys C,BUN and SCr in observation group were significantly lower than those in control group(P<0.05),there was no significant difference in blood pressure and lipid levels between the two groups(P>0.05).Conclusion Buliu Lishen Decoction combined with Bailing Capsule can reduce blood sugar levels in patients with early diabetic nephropathy,and improve blood pressure,blood lipids and renal function and reduce HCY MAU and UAER levels,and has high clinical efficacy.
