扶正平瘤汤联合手术治疗脑胶质瘤临床疗效研究

Clinical Effect of Fuzheng Pingliu Decoction Combined with Surgery in Treatment of Glioma

目的观察扶正平瘤汤联合手术治疗脑胶质瘤临床疗效及对术后复发的影响。方法选取我院收治的104例脑胶质瘤患者,采用随机数值表法分为观察组与对照组,最终观察组50例、对照组49例完成本次研究。两组患者均予手术治疗,对照组术后予替莫唑胺胶囊治疗3个疗程,观察组在此基础上加予扶正平瘤汤治疗3个疗程。于治疗前后,采用中医症状积分量表进行头痛、头晕、食少纳呆、肢体困重积分计算,采用卡氏评分评估生活质量情况,采用流式细胞仪检测患者全血中CD4+、CD8+比例,计算CD4+/CD8+。评估两组药物服用期间毒副反应,并于治疗结束后评估两组临床疗效,随访1年,统计术后复发率。结果治疗后,观察组的总有效率、生活质量改善有效率均明显高于对照组(P<0.05);两组患者头痛、头晕、食少纳呆、肢体困重的中医症候积分均较治疗前明显下降(P<0.05);且观察组治疗后上述各中医症候积分均明显低于对照组治疗后(P<0.05)。观察组治疗后CD4+、CD4+/CD8+明显高于对照组治疗后,CD8+明显低于对照组治疗后(P<0.05)。治疗期间,观察组发生消化道不良反应例数明显少于对照组(P<0.05),且观察组随访1年复发率明显少于对照组(P<0.05)。结论扶正平瘤汤联合手术治疗脑胶质瘤患者可明显提高临床疗效,提升患者免疫力及生活质量,降低术后复发率,且治疗安全性较高。

Objective To observe the clinical effect of Fuzheng Pingliu Decoction combined with surgery in treatment of glioma and its influences on postoperative recurrence.Methods A total of 104 glioma patients were randomly divided into observation group(n=50)and control group(n=49).Both groups were treated with surgery and were given temozolomide for3 courses after surgery.On this basis,observation group was treated with Fuzheng Pingliu Decoction for 3 courses.The scores of headache,dizziness,and anorexia and limbs heaviness were calculated by TCM Symptom Scale Before and after treatment;the quality of life was evaluated by Kamofsky Performance Status.The proportions of CD4^+and CD8^+in the whole blood were detected by flow cytometry,and CD4^+/CD8^+were calculated,toxicity reactions during medication in both groups were evaluated.The clinical curative effects in both groups after treatment were evaluated.The follow-up lasted for 1 year to statistically analyze the postoperative recurrence rate.Results After treatment,the total response rate and response rate of life quality improvement in observation group were significantly higher than those in control group(P<0.05);scores of TCM symptoms such as headache,dizziness anorexia and limbs heaviness were significantly decreased in both groups(P<0.05);the scores of the above TCM symptoms in observation group were significantly lower than those in control group(P<0.05);after treatment,CD4^+and CD4^+/CD8^+in observation group were significantly higher than those in control group,while CD8^+was significantly lower than that in control group(P<0.05).During medication,the number of cases with adverse gastrointestinal reactions in observation group was significantly less than that in control group(P<0.05),and the recurrence rate after 1 year of follow-up was significantly lower than that in control group(P<0.05).Conclusion Fuzheng Pingliu Decoction combined with surgery for glioma patients can significantly improve the clinical curative effect,enhance their immunity and quality of life,and reduce postoperative recurrence rate,with relatively higher treatment safety.
