脑血疏联合尼莫地平对自发性蛛网膜下腔出血脑血管痉挛和迟发性脑缺血的影响

Effects of Cerebral Hemolysis and Nimodipine on Cerebral Vascular Spasm and Delayed Cerebral Ischemia in Spontaneous Subarachnoid Hemorrhage

目的观察脑血疏联合尼莫地平对自发性蛛网膜下腔出血脑血管痉挛和迟发性脑缺血的影响。方法选取在我院接受治疗的自发性蛛网膜下腔出血(SAH)患者78例为研究对象,应用随机数字表法分为观察组与对照组各39例。对照组采用尼莫地平进行治疗,观察组采用尼莫地平联合脑血疏口服液进行治疗,比较两组临床疗效、美国国立卫生所卒中量表(NIHSS),修正Rankin量表评分(MRS)及脑血管痉挛(CVS)、迟发性脑缺血(DCI)的发生率。结果观察组总有效率显著高于对照组(P<0.05)。治疗后两组NIHSS、MRS评分均下降(P<0.05);观察组NIHSS、MRS评分均低于对照组(P<0.05)。观察组CVS发生率低于对照组;观察组DCI发生率低于对照组;观察组总死亡率为低于对照组(P<0.05)。结论脑血疏联合尼莫地平对自发性蛛网膜下腔出血疗效较好,明显改善患者神经功能和致残率,减少不良脑血管事件的发生。

Objective To observe the effects of cerebral hemorrhage combined with nimodipine on cerebral vasospasm and delayed cerebral ischemia in spontaneous subarachnoid hemorrhage(SAH). Methods 78 cases of patients with SAH wererandomly divided into observation group and control grou,39 cases each. Control group was treated with nimodipine,observation group was treated with nimodipine combined with Naoxueshu oral solution. The clinical efficacies of the two groups were compared,as well as the National Institutes of Health Stroke Scale(NIHSS),the Rankin Scale Score(MRS),and Incidence of cerebral vasospasm(CVS)and delayed cerebral ischemia(DCI). Results The total effective rate in observation group was significantly higher than that in control group( P<0.05);the NIHSS and MRS scores of both groups decreased after treatment( P<0.05);the NIHSS and MRS scores of observation group were lower than those of control group after treatment( P<0.05);the incidence of CVS in observation group was lower than that of control group;the incidence of DCI in observation group was lower than control group;the total mortality in observation group was lower than that in control group( P<0.05). Conclusion Cerebral Xueshu combined with nimodipine has a good effect on SAH,which significantly improves the patients’ neurological function and disability,and reduces the occurrence of adverse cerebrovascular events.
