减毒抑癌汤联合TC方案治疗晚期卵巢癌临床疗效及与T细胞亚群水平关系的研究

Clinical Efficacy of Jiandu Yiai Decoction Combined with TC Regimen in Treatment of Advanced Ovarian Cancer and Its Effect on T Cell Subsets

目的观察减毒抑癌汤联合TC(紫杉醇+卡铂)方案治疗晚期卵巢癌临床疗效及其对T细胞亚群水平的影响。方法选择气虚血瘀型卵巢癌患者80例,随机分为两组,对照组接受TC方案化疗,研究组接受TC化疗联合减毒抑癌汤治疗,两组均接受4个疗程的治疗。比较两组的临床疗效,检测比较两组的免疫水平、生活质量评分以及不良反应发生情况。结果治疗后,研究组的客观缓解率显著高于对照组(P<0.05);两组糖类抗原199(CA199)和癌胚抗原(CEA)水平均显著降低(P<0.05),研究组的CA199和CEA显著低于对照组(P<0.05)。治疗后,两组的T细胞亚群分布均有显著改善(P<0.05),研究组的CD3^+、CD4^+以及CD4^+CD8^+显著高于对照组(P<0.05)。治疗后,两组EORTCQLQ-C30和中医症候评分均显著好转(P<0.05),研究组EORTCQLQ-C30评分显著高于对照组而中医症候评分显著低于对照组(P<0.05)。治疗后,研究组白细胞降低、贫血、血小板减少以及关节肌肉痛的发生率显著低于对照组(P<0.05)。结论减毒抑癌汤联合TC化疗对卵巢癌具有更好的治疗效果,并且可以有效改善免疫水平,减少并发症的发生。

Objective To observe the clinical efficacy of Jiandu Yiai decoction combined with TC(paclitaxel+carboplatin)regimen in treatment of advanced ovarian cancer and its effect on the level of T cell subsets.Methods 80 cases of patients with qi deficiency and blood stasis type of ovarian cancer were randomly divided into study group and control group.Control group received chemotherapy with TC regimen,study group received TC chemotherapy combined with attenuated tumor suppressor therapy.Both groups received 4 courses of treatment.The clinical efficacy of the two groups was compared,and the immune level,quality of life score and adverse reactions were detected and compared between the two groups.Results After treatment,the objective response rate of study group was significantly higher than that of control group(P<0.05);the carbohydrate antigen199(CA199)and carcino-embryonic antigen(CEA)of study group were significantly lower than those of control group(P<0.05).There was no significant difference in the distribution of T cell subsets between the two groups before treatment(P>0.05),after treatment,both groups showed significant improvement(P<0.05).The CD3^+,CD4^+and CD4^+/CD8^+in study group were significantly higher than those in control group(P<0.05).The EORTCQLQ-C30 and TCM symptom scores were significantly improved(P<0.05),the EORTCQLQ-C30 score of study group was significantly higher than that of control group and the TCM symptom score was significantly lower than that of control group(P<0.05).The incidence of leukopenia,anemia,thrombocytopenia,and joint muscle pain was significantly lower in study group than those in control group(P<0.05).Conclusion Attenuated tumor suppressant combined with TC chemotherapy has a better therapeutic effect on ovarian cancer,and can effectively improve the immune level and reduce the occurrence of complications.
