DCE-MRI定量参数与子宫内膜癌患者浸润程度的相关性

摘要 目的分析动态增强磁共振成像(DCE-MRI)定量参数与子宫内膜癌患者浸润程度的相关性。方法选取2019年1月—2020年12月长葛市人民医院收治的90例子宫内膜癌患者,入院时均行DCE-MRI检查,获得定量参数,以组织病理学检查结果为依据,记录子宫内膜癌患者肌层浸润程度并分组;比较不同肌层浸润程度患者DCE-MRI定量参数[容量转移常数(K^(trans))、速率常数(K_(ep))、血管外细胞外间隙容积比(K_(e))],分析DCE-MRI定量参数对子宫内膜癌患者浸润程度的评估价值。结果90例子宫内膜癌患者中,无/浅肌层浸润49例,深肌层浸润41例;深肌层浸润组K^(trans)、K_(e)值分别为(0.51±0.16)min、(0.58±0.17)min均高于无/浅肌层浸润组的(0.35±0.11)min、(0.46±0.13)min,差异有统计学意义(P<0.05);深肌层浸润组K_(ep)值为(0.91±0.28)min,无/浅肌层浸润组K_(ep)值为(0.82±0.25)min,两组K_(ep)值比较,差异无统计学意义(P>0.05);绘制受试者工作曲线(ROC),结果显示,K^(trans)、K_(e)单独及联合评估子宫内膜癌患者深肌层浸润程度的曲下面积(AUC)分别为0.808、0.770、0.889,评估价值较理想,且以联合评估价值最佳。结论DCE-MRI定量参数中K^(trans)、K_(e)与子宫内膜癌患者肌层浸润程度有关,对评估深肌层浸润具有一定价值。 Objective To analyze the correlation between quantitative parameters of dynamic contrast enhanced magnetic resonance imaging(DCE-MRI) and the degree of invasion in patients with endometrial cancer.Methods Totally 90 patients with endometrial cancer treated in Changge People’s Hospital from January 2019 to December 2020 were selected, DCE-MRI was performed at admission to obtain quantitative parameters, according to the results of histopathological examination, the degree of myometrial invasion in patients with endometrial cancer was recorded and divided into groups;The quantitative parameters of DCE-MRI[volume transfer constant(K^(trans)), rate constant(K_(ep)), extracellular space volume ratio(K_(e))]were compared in patients with different degrees of myometrial infiltration, the value of DCE-MRI quantitative parameters in evaluating the degree of invasion in patients with endometrial cancer was analyzed.Results Among 90 cases of patients with endometrial carcinoma, 49 cases had no/superficial myometrial invasion and 41 cases had deep myometrial invasion;The values of K^(trans) and K_(e) in deep muscle infiltration group were(0.51±0.16) min and(0.58±0.17) min, respectively, which were higher than those in no/superficial muscle infiltration group(0.35±0.11) min and(0.46±0.13) min, the difference was statistically significant(P<0.05);The K_(ep) value of deep muscle infiltration group was(0.91±0.28) min, and that of no/superficial muscle infiltration group was(0.82±0.25) min, there was no statistical significant difference in K_(ep) between the two groups(P>0.05);The receiver operating curve(ROC) was drawn, and the results showed that the area under the curve(AUC) of K^(trans) and K_(e) alone and in combination got evaluating the degree of myometrial invasion in patients with endometrial cancer were 0.808, 0.770 and 0.889 respectively, the evaluation value was ideal, and the joint evaluation value was the best.Conclusion K^(trans) and K_(e) in the quantitative parameters of DCE-MRI are related to the degree of myometrial invasion in patients with endometrial cancer, and have certain value in evaluating deep myometrial invasion.
