uf-5000在评估留置导尿管患者尿路感染中的应用价值

Application value of uf-5000 automatic urine sediment analyzer in diagnosis of bacterial urinary tract infection in inpatients with indwelling catheter

摘要 目的评估uf-5000全自动尿沉渣分析仪细菌计数结果在诊断住院留置导尿管患者细菌性尿路感染中的应用价值。方法收集2020年7—10月河南省人民医院部分疑似尿路感染住院患者留置导尿管尿标本225例,同时进行尿液细菌定量培养及uf-5000全自动尿沉渣分析仪检测细菌数。以尿液细菌定量培养结果为金标准,对两种检测方法的细菌计数和革兰氏分类结果进行比较,同时绘制受试者工作特征曲线(ROC曲线)评估uf-5000全自动尿沉渣分析仪细菌计数结果的诊断能力。结果225例住院患者尿标本中有71例尿细菌培养阳性(31.6%),154例阴性(68.4%)。uf-5000全自动尿沉渣分析仪尿细菌计数对尿路感染诊断价值较高,cut-off值为71个/μL时,其灵敏度和特异性分别为94.4%和90.9%,ROC曲线下面积为0.965;uf-5000全自动尿沉渣分析仪鉴定革兰氏阴性菌的符合率为82.0%,鉴定革兰氏阳性菌的符合率为61.9%。结论用uf-5000全自动尿沉渣分析仪对尿液中细菌进行准确计数对于住院留置导尿管患者的尿路感染可能有比较好的应用价值,但是关于其细菌革兰氏菌群分类的准确性有待提高。 Objective To evaluate the application value of uf-5000 automatic urine sediment analyzer in the diagnosis of bacterial urinary tract infection in inpatients with indwelling catheter.Methods A total of 225 urine specimens with indwelling catheters were collected from some inpatients suspected of urinary tract infection in Henan Provincial People’s Hospital from July to October 2020.At the same time,urine bacteria were quantitatively cultured and detected by uf-5000 automatic urine sediment analyzer.With the results of urine bacterial quantitative culture as the gold standard,the bacterial count and gram classification results of the two detection methods were compared,and the receiver operating characteristic curve(ROC curve)was drawn to evaluate the diagnostic ability of the bacterial count results of uf-5000 automatic urine sediment analyzer.Results Among 225 inpatient urine samples,71 cases(31.6%)were positive for bacterial culture,and 154 cases(68.4%)were negative.The uf-5000 automatic urine sediment analyzer had a high value in the diagnosis of urinary tract infection.When the cut-off value was 71/μL,the sensitivity and specificity were 94.4%and 90.9%,respectively,and the area under the ROC curve was 0.965.The coincidence rate of gram-negative bacteria and Gram-positive bacteria identified by uf-5000 automatic urine sediment analyzer was 82.0%and 61.9%respectively.Conclusion Accurate counting of bacteria in urine by uf-5000 automatic urine sediment analyzer may have good application value for urinary tract infection in inpatients with indwelling catheter,but the accuracy of bacterial gram flora classification needs to be improved.
