小骨窗经外侧裂微创术对基底节HICH患者血清S100B蛋白、NSE水平的影响 被引量:1

Effect of small bone window minimally invasive surgery through lateral fissure on serum S100B protein and NSE levels in patients with HICH in basal ganglia

摘要 目的探讨基底节高血压性脑出血(HICH)患者采用小骨窗经外侧裂微创术治疗对其血清S100B蛋白、神经元特异性烯醇化酶(NSE)水平的影响。方法前瞻性选择2019年2月—2020年7月驻马店市中医院收治的74例基底节HICH患者,采用随机数字表法分为对照组与观察组,各37例。对照组患者采用大骨瓣经颞叶入路开颅术进行治疗,观察组患者采用小骨窗经外侧裂微创术进行治疗。比较两组患者术前、术后1周神经组织损伤指标[S100B蛋白、NSE]水平及手术时间。结果术后1周,两组患者S100B蛋白、NSE水平均低于术前,且观察组S100B蛋白[(0.32±0.09)μg/L]、NSE[(13.46±1.57)ng/mL]水平低于对照组[(0.41±0.10)μg/L、(15.86±1.62)ng/mL],差异有统计学意义(P<0.05);观察组患者手术时间[(2.16±0.33)h]短于对照组[(4.35±0.84)h],差异有统计学意义(P<0.05)。结论小骨窗经外侧裂微创术治疗基底节HICH,可改善患者血清S100B蛋白、NSE水平,缩短手术时间。 Objective To investigate the effect of small bone window minimally invasive surgery through lateral fissure on serum S100 B protein and neuron-specific enolase(NSE)levels in patients with hypertensive intracerebral hemorrhage(HICH)in basal ganglia.Methods Totally 74 patients with HICH in basal ganglia treated in Zhumadian Traditional Chinese Medicine Hospital from February 2019 to July 2020 were prospectively selected,a random number table method was used to divide them into an observation group and a control group,37 cases in each group.The control group was received large bone flap craniotomy through temporal lobe approach for treatment,and the observation group was received small bone window minimally invasive surgery through lateral fissure for treatment.The indexes of nerve tissue injury[S100 B protein,NSE]and operation time were compared between the two groups before and 1 week after operation.Results One week after operation,S100 B protein and NSE levels of patients in both groups were decreased,and the S100 B protein[(0.32±0.09)μg/L]and NSE[(13.46±1.57)ng/mL]in the observation group were lower than[(0.41±0.10)μg/L,(15.86±1.62)ng/mL]in the control group,with statistical significant differences(P<0.05);The operation time of patients in the observation group[(2.16±0.33)h]was shorter than the control group[(4.35±0.84)h],with a statistical significant difference(P<0.05).Conclusion Small bone window minimally invasive surgery through lateral fissure in the treatment of HICH in basal ganglia can improve the serum S100 B protein and NSE levels of patients,and reduce the operation time.
