血清HIF-1α、CRP水平与溃疡性结肠炎患者美沙拉秦联合益生菌治疗效果的相关性

Correlation between serum HIF-1 α,CRP levels and treatment effect of Mesalazine combined with probiotics in patients with ulcerative colitis

目的探讨血清低氧诱导因子-1α(HIF-1α)、C反应蛋白(CRP)水平与溃疡性结肠炎(UC)患者美沙拉秦联合益生菌治疗效果的相关性。方法选择2018年6月—2020年6月郑州大学第一附属医院收治的UC患者81例,均接受美沙拉秦联合益生菌治疗,在治疗4周后进行疗效评估。于治疗前检测所有患者血清HIF-1α、CRP水平,分析上述指标与UC患者美沙拉秦联合益生菌治疗效果的相关性;并绘制受试者工作曲线(ROC),检验血清HIF-1α、CRP水平预测UC患者美沙拉秦联合益生菌治疗无效的风险价值。结果无效组血清HIF-1α及CRP水平均高于有效组(P<0.05)。经Logistic回归分析结果显示,血清HIF-1α、CRP水平过高表达可能是UC美沙拉秦联合益生菌治疗无效的影响因素(OR>1,P<0.05)。绘制ROC曲线,结果显示,治疗前血清HIF-1α、CRP水平预测UC患者经美沙拉秦联合益生菌治疗无效风险的AUC>0.80,预测价值较理想,且联合预测的效果更佳。依据ROC曲线得出治疗前血清HIF-1α、CRP水平的cut-off值分别为63.87 ng/L、8.40 mg/L,在该cut-off值水平下,取特异度、敏感度较理想。结论血清HIF-1α、CRP高表达会导致UC患者经美沙拉秦联合益生菌治疗无效,二者治疗前表达情况对预测UC患者经美沙拉秦联合益生菌治疗效果有一定价值,临床可加强对上述水平的监测,及时进行干预,以提高治疗效果。

Objective To investigate the correlation between the levels of serum hypoxia inducible factor-1α(HIF-1α)and C-reactive protein(CRP)and the treatment effect of mesalazine combined with probiotics in patients with ulcerative colitis(UC).Methods Totally 81 patients with UC admitted to our hospital from June 2018 to June 2020 were selected,all patients were treated with mesalazine combined with probiotics,the efficacy was evaluated after 4 weeks of treatment.Serum HIF-1αand CRP levels of all patients were detected before treatment,the correlation between the above indexes and the treatment effect of mesalazine combined with probiotics in UC patients was analyzed,the receiver operating curve(ROC)was drawn,the value of serum HIF-1α,CRP levels in predicting the risk of ineffective treatment of mesalazine combined with probiotics in UC patients was tested.Results The levels of serum HIF-1αand CRP in the ineffective group were higher than those in the effective group(P<0.05).The results of Logistic regression analysis showed that the over expression of serum HIF-1αand CRP may be the influencing factors of ineffective treatment of UC with mesalazine and probiotics(OR>1,P<0.05);The ROC curve was drawn,and the results showed that the AUC of serum HIF-1α,CRP levels before treatment in predicting the risk of ineffective treatment of mesalazine combined with probiotics in UC patients were>0.80,the predictive value was ideal,and the effect of combined prediction was better.According to the ROC curve,the cut-off values of the serum HIF-1α,CRP levels before treatment were 63.87 ng/L and 8.40 mg/L,respectively,at this cut-off value level,the specificity and sensitivity were ideal.Conclusion The high expression of serum HIF-1α,CRP levels will lead to ineffective treatment of mesalazine combined with probiotics in UC patients,the expression of the two before treatment has certain value in predicting the therapeutic effect of mesalazine combined with probiotics in UC patients,in order to improve the therapeutic effect,the monitoring of the above levels should be strengthened in clinic and intervened in time.
