独活寄生汤联合传统椎板开窗术治疗风寒湿痹型腰椎间盘突出症的疗效观察

Effect of Duhuo Jisheng decoction combined with traditional laminectomy in treatment of lumbar disc herniation of wind-cold-dampness syndrome

目的观察独活寄生汤联合传统椎板开窗术治疗风寒湿痹型腰椎间盘突出症(LDH)的疗效。方法2018年3月—2020年3月中国人民解放军联勤保障部队第九八八医院收治的70例LDH患者,根据随机数字表法分为治疗组和对照组,每组35例。对照组仅行传统椎板开窗术治疗,治疗组行传统椎板开窗术配合独活寄生汤治疗,术后1个月、术后3个月,观察两组腰痛VAS评分、腿痛VAS评分、ODI及临床疗效等指标的变化情况。结果治疗组和对照组术后1个月、3个月时腰痛VAS评分均比术前下降,差异有统计学意义(F=966.321,741.451;P=0.000,0.000)。治疗组和对照组术后1个月、3个月时腿痛VAS评分均比术前下降,差异有统计学意义(F=2432.924,1611.348;P=0.000,0.000)。治疗组和对照组术后1个月、3个月时ODI均比术前下降,差异有统计学意义(F=574.560,520.831;P=0.000,0.000)。治疗组术后1个月的腰痛VAS评分、腿痛VAS评分、ODI改善情况均优于对照组,差异有统计学意义(t=7.685,5.542,8.189;P=0.000,0.000,000)。治疗组术后3个月的腰痛VAS评分、腿痛VAS评分、ODI改善情况均优于对照组,差异有统计学意义(t=2.741,4.318,6.056;P=0.008,0.000,000)。术后3个月治疗组总有效率94.29%(33/35)优于对照组总有效率80.00%(28/35),差异有统计学意义(Z=2.113,P=0.035)。结论独活寄生汤联合传统椎板开窗术治疗风寒湿痹型LDH在改善腰腿痛、腰椎功能、术后疗效方面优于单纯的传统椎板开窗术。

Objective To observe the effect of Duhuo Jisheng decoction combined with traditional laminectomy in the treatment of wind-cold-dampness syndrome lumbar disc herniation(LDH).Methods From March 2018 to March 2020,70 patients with LDH were divided into treatment and control groups according to the random number table method,with 35 cases in each group.The control group was only treated with traditional laminectomy,and the treatment group was treated with traditional laminectomy with Duhuo Jisheng decoction,and the changes of indicators such as VAS score of low back pain,VAS score of leg pain,ODI and clinical efficacy were observed in the two groups after 1 month and 3 months after surgery.Results Both the treatment group and the control group had lower back pain VAS scores at 1 month and 3 months after surgery than before the operation(F=966.321,741.451;P=0.000,0.000).The VAS scores of leg pain at 1 month and 3 months after operation in the treatment group and the control group were lower than those before the operation,and there was a significant difference(F=2432.924,1611.348;P=0.000,0.000).The ODI of the treatment group and the control group decreased at 1 month and 3 months after the operation,and there was a significant difference(F=574.560,520.831;P=0.000,0.000).The improvement of back pain VAS score,leg pain VAS score,and ODI 1 month after operation in the treatment group were better than those of the control group,and the difference was statistically significant(t=7.685,5.542,8.189;P=0.000,0.000,000).The improvement of low back pain VAS score,leg pain VAS score,and ODI in the treatment group 3 months after operation were better than those of the control group,and the difference was statistically significant(t=2.741,4.318,6.056;P=0.008,0.000,000).Three months after operation,the total effective rate of the treatment group was 94.29%(33/35)better than the control group’s total effective rate of 80.00%(28/35),the difference was statistically significant(Z=2.113,P=0.035).Conclusion Duhuo Jisheng decoction combined with traditional laminectomy in the treatment of wind-cold-dampness syndrome lumbar disc herniation is better than traditional laminectomy in improving low back pain,lumbar function and postoperative curative effect.
