丁苯酞注射液治疗前循环急性脑梗死的临床分析

Clinical evaluation of Butylphthalide injection in treatment of anterior circulation acute cerebral infarction

目的观察丁苯酞注射液治疗前循环急性脑梗死的疗效,分析用药安全性。方法纳入120例未行溶栓或取栓的前循环急性脑梗死患者,随机分为对照组和治疗组各60例。所有患者给予脑血管病规范治疗,治疗组加用丁苯酞注射液。随访90 d,比较14 d后美国国立卫生研究院卒中量表(NIHSS)评分、90 d后改良Rankin量表(mRS)评分;安全性指标为药物相关严重不良反应及90 d症状性卒中、心肌梗死、全因死亡的发生率。结果14 d后治疗组NIHSS评分改善情况明显优于对照组(P<0.05);治疗组90 d实现功能独立(mRS≤2分)患者显著多于对照组(P<0.05)。随访期间2组患者均无严重药物不良反应,2组症状性脑卒中、心肌梗死、全因死亡的发生情况无显著差异(P>0.05)。结论前循环急性脑梗死患者早期使用丁苯酞注射液可以改善临床症状及远期预后,安全性良好。

Objective To evaluate the clinical value of Butylphthalide injection in treatment of anterior circulation acute cerebral infarction.Methods Totally 120 acute cerebral infarction patients were randomly divided into treatment group and control group at ratio of 1:1.All patients received basic treatment,while patients of the treatment group received additional Butylphthalide injection.The National Institutes of Health stroke scale(NIHSS)score 14 days later and the modified Rankin scale(mRS)scores of 90 days later were considered as efficacy index.Patients who had gone through severe adverse effects during the treatment or symptomatic stroke,myocardial infarction or died for all reasons during 90 days follow-up were recorded and these data was used for safety estimate.Results Fourteen days after treatment,patients of both groups had significant lower NIHSS score,while the treatment group had significantly lower NIHSS score(P<0.05).A 90 days span follow-up were performed after treatment.Treatment group subjects got statistically lower mRS score(P<0.05).No significant difference of severe adverse effects,symptomatic stroke,myocardial infarction or mortality was found between the two groups(P>0.05).Conclusion Butylphthalide injection can not only improve the clinical symptoms at the early stage,but also improve long-term prognosis of anterior circulation acute cerebral infarction patients.Butylphthalide injection may be safe and effective for anterior circulation stroke patients.
