对小儿动脉导管未闭介入治疗术中进行心理干预的效果分析

Analysis of effect of psychological intervention during interventional therapy for patent ductus arteriosus in children

目的探讨小儿动脉导管未闭(patent ductus arteriosus,PDA)介入治疗术中实施心理干预对手术时间和术中并发症的影响。方法回顾2018年6月—2020年6月在河南省人民医院介入手术室接受PDA介入治疗的患儿(3~6岁)50例,分为A、B两组,A组为家属陪护组,男8例,女17例,平均体重(15.43±2.32)kg,B组为无家属陪护组,男11例,女14例,平均体重(15.40±3.12)kg。两组手术均由同一术者完成,记录手术时间和术中并发症出现概率并作统计学分析。结果50例患儿均顺利完成手术,统计显示,两组患儿的年龄、体重及性别差异无统计学意义(年龄:t=1.914,P=0.060;体重:t=1.705,P=0.094;性别:χ2=3.00,P=0.082;手术时间相差约1.07倍[(3.25~6.76)/3.25],差异有统计学意义(t=5.337,P=0.000);术中并发症出现概率差异有统计学意义。结论心理干预在PDA介入治疗术中可部分缓解患儿的疼痛和恐惧,对药物镇静具有补充作用,缩短了手术时间,减少了并发症的发生。

Objective To investigate the effect of psychological intervention on operation time and complications during interventional therapy for patent ductus arteriosus(PDA)in children.Methods Total of 50 children(3-6 years old)receiving PDA intervention in the He Nan Provincial People′Hospital from June 2018 to June 2020 were reviewed and divided into two groups:group A 8 males and 17 females,with an average weight of(15.43±2.315)kg and group B 11 males and 14 females,with an average weight of(15.40±3.156)kg.Two groups of operations were completed by the same surgeon,and the operation time and the probability of intraoperative complications were recorded and statistically analyzed.Results Totally 50 cases of children were successfully completed surgery,statistics showed that two groups of children with age,weight and gender difference was not statistically significant(age:t=1.914,P=0.060;Weight:t=1.705,P=0.094;Gender:χ2=3.00,P=0.082;The difference in operation time was about 1.07 times[(3.25~6.76)/3.25],and the difference was statistically significant(t=5.337,P=0.000);The difference in the probability of intraoperative complications was statistically significant.Conclusion Psychological intervention can partially relieve the pain and fear of children during PDA interventional therapy,which has a supplementary effect on drug sedation,shortens the operation time and reduces the occurrence of complications.
