无痛穴位埋线联合失笑芍甘汤治疗寒凝血瘀证原发性痛经临床疗效观察

Clinical observation of painless acupoint embedding combined with Shixiao Shaogan decoction in treatment of primary dysmenorrhea with cold pain syndrome

目的观察无痛穴位埋线联合失笑芍甘汤治疗寒凝血瘀证原发性痛经(PD)的临床疗效。方法将2019年4月—2021年3月在河南中医药大学第三附属医院门诊收治的122例原发性痛经(寒凝血瘀证)患者随机分为两组:治疗组61例采用穴位(肾俞、次髎、三阴交)无痛埋线联合失笑芍甘汤中药内服治疗;对照组61例口服布洛芬缓释胶囊治疗。观察治疗后有效率、VAS评分、血清β-内啡肽(β-EP)及前列腺素E2(PGE2)的变化及治疗不良反应。结果两组经过治疗后,治疗组和对照组的总有效率分别为95.33%和80.33%,经统计分析,两组差异有统计学意义(P<0.05)。两组治疗后VAS评分进行比较,近期VAS评分(P>0.05),表明两组差异无统计学意义,而远期P<0.05,在VAS评分改善方面差异有统计学意义,治疗组在VAS评分改善方面明显优于对照组。治疗后,2组β-EP及PGE2水平均较治疗前上升(P<0.05);治疗组β-EP、PGE2水平均高于对照组(P<0.05)。在治疗不良反应方面,对照组使用布洛芬缓释胶囊因部分患者出现恶心、呕吐及腹泻等情况不能继续服用,而无痛穴位埋线联合失笑芍甘汤对此类患者能适用,二者相比较具有绝对优势。结论无痛穴位埋线联合失笑芍甘汤治疗寒凝血瘀证原发性痛经疗效显著,无不良反应。

Objective To observe the clinical effect of painless acupoint embedding combined with Shixiaoshaogan decoction on primary dysmenorrhea(PD)with syndrome of cold coagulation and blood stasis.Methods A total of 122 patients with primary dysmrhea(syndrome of cold coagulation and blood stasis)admitted to the Third Affiliated Hospital of Henan University of Chinese Medicine from April 2019 to March 2021 were randomly divided into two groups:61 patients in the treatment group were treated with painless catemetery at acupoints(Shenshu,Ciliao and Sanyinjiao)combined with Shixiaoshaogan decoction;61 patients in the control group were treated with oral ibuprofen sustained release capsules.The effective rate after treatment,VAS score,serumβ-endorphin(β-EP)and prostaglandin E2(PGE2)were observed and adverse reactions were treated.Results After treatment,the total effective rate of the treatment group and the control group was 95.33%and 80.33%.After statistical analysis,there was a significant difference between the two groups(P<0.05).The VAS scores of the two groups after treatment were compared.The recent VAS score was P>0.05,indicating that there was no difference between the two groups,but the long-term P<0.05,there was significant statistical difference in the improvement of VAS score.The treatment group was superior in the improvement of VAS score.In the control group.After treatment,the levels ofβ-EP and PGE2 in the two groups were higher than those before treatment(P<0.05).The levels ofβ-EP and PGE2 in the treatment group were higher than those in the control group(P<0.05).In the treatment of adverse reactions,the control group using ibuprofen sustained-release capsules due to some patients with nausea,vomiting and diarrhea can not continue to take,and painless acupoint embedding needle combined with Shixiaoshaogan decoction can be applied to such patients,which has an absolute advantage.Conclusion The painless acupoint embedding combined with Shixiao shaogan decoction for the treatment of cold clotting syndrome has significant curative effect and no adverse reactions.
