血脂紊乱对妊娠期糖尿病患者胰岛素抵抗的影响

Effect of dyslipidemia on insulin resistance in gestational diabetic women

目的研究血脂紊乱对妊娠期糖尿病患者胰岛素抵抗的影响。方法在南阳市中心医院中选取2018年1月—2019年12月符合妊娠期糖尿病症状的162例患者,根据患者的胰岛素抵抗指数(HOMA-IR)是否大于2.69,将HOMA-IR大于2.69的患者分为胰岛素抵抗高水平组,将HOMA-IR小于等于2.69的患者分为胰岛素抵抗低水平组,对以上两组患者均进行低密度脂蛋白胆固醇(LDL-C)、总胆固醇(TC)、三酰甘油(TG)及高密度脂蛋白胆固醇(HDL-C)水平检测,并对患者计算后的HOMA-IR指数进行统计,分析两组患者血脂紊乱发生率与妊娠期糖尿病妇女胰岛素抵抗程度的相关性,且分析各项血脂水平检测对妊娠期糖尿病妇女胰岛素抵抗程度的影响情况。结果胰岛素抵抗低水平组患者血脂情况较为良好,血脂紊乱发生率低于胰岛素抵抗高水平组患者,胰岛素抵抗高水平组患者总体血脂紊乱发生率为69.8%(113/162),显著高于胰岛素抵抗低水平组[13.33%(8/60)],且HOMA-IR水平与HDL-C水平具有正向相关性,相关系数为0.211,与TC、TG及LDL-C水平成负相关,相关系数分别为-0.157、-0.258、-0.137,差异无统计学意义(P>0.05);对以上两组患者采用线性回归模型分析,结果显示,患者胰岛素抵抗程度与TC、HDL-C分别为独立相关,相关性系数t分别为4.289、3.544,差异无统计学意义(P>0.05)。结论妊娠期糖尿病妇女胰岛素抵抗程度与血脂紊乱情况具有一定的关联度,患者胰岛素抵抗程度与TC、HDL-C具有独立相关性。

Objective To study the effect of dyslipidemia on insulin resistance in gestational diabetic women.Method In the hospital,162 women with gestational diabetes symptoms from January 2018 to December 2019 were selected.According to whether their HOMA-IR was greater than 2.69,the patients with HOMA-IR greater than 2.69 were divided into high-level insulin resistance group,and the patients with HOMA-IR less than or equal to 2.69 were divided into low-level insulin resistance group.The above two groups were treated with low-density lipoprotein The levels of LDL-C,TC,TG and HDL-C were measured and calculated HOMA-IR index was used to analyze the relationship between the incidence of dyslipidemia and the degree of insulin resistance in gestational diabetic women.Results The incidence of dyslipidemia in low insulin resistance group was lower than that in high insulin resistance group 69.8%(113/162),significantly higher than 13.33%(8/60)in the low-level group of insulin resistance,and HOMA-IR level has a positive correlation with HDL-C level,with a correlation coefficient of 0.211,and a negative correlation with TC,TG and LDL-C levels,with a correlation coefficient of-0.157,-0.258,-0.137 respectively,there was no significant difference between the two groups(P>0.05).Linear regression analysis showed that the degree of insulin resistance was independently correlated with TC and HDL-C,and the correlation coefficients were 4.289 and 3.544,respectively(P>0.05).Conclusion There is a certain correlation between insulin resistance and dyslipidemia in gestational diabetic women,and there is an independent correlation between insulin resistance and TC,HDL-C.
