肿瘤体积对胃肠间质瘤治疗疗效的评估价值

Value of tumor volume in assessing the therapeutic efficacy of gastrointestinal stromal tumor

摘要 目的探讨肿瘤体积指标对胃肠间质瘤(gastrointestinal stromal tumors,GIST)患者药物治疗疗效的评估价值。方法回顾性分析哈尔滨医科大学附属肿瘤医院2017年5月~2021年10月经病理证实的胃肠间质瘤患者30例,在治疗期间行两次增强CT检测,测量病灶最长径、肿瘤体积的变化。分析肿瘤体积变化率与RECIST评分和Choi评分的相关性。绘制受试者工作特征曲线(receiver operating characteristic curve,ROC曲线)分析瘤体积变化率与患者药物治疗效果的相关性。结果肿瘤体积变化率与RECIST评分标准呈强相关(r=0.646,P<0.01),与Choi评分标准呈中等相关(r=0.498,P<0.01)。ROC曲线中曲线下面积(area under curve,AUC)为0.847(95%CI:0.679~1.000),P<0.05。结论肿瘤体积有助于评估胃肠间质瘤患者药物治疗疗效,对于药物治疗时间的把控及手术时机的决策具有重要的指导意义。 Objective To explore the value of tumor volume index in the evaluation of therapeutic efficacy of medication for patients with gastrointestinal stromal tumors(GIST).Methods Thirty patients with GIST confirmed by pathology in the Harbin Medical University Cancer Hospital from May 2017 to October 2021 were analyzed retrospectively.During the treatment,enhanced CT was performed twice to measure the changes of the longest diameter of the focus and tumor volume.The correlation between tumor volume change rate and RECIST score and Choi score was analyzed.Receiver operating characteristic(ROC)curve was drawn to analyze the correlation between tumor volume change rate and drug treatment effect.Results The change rate of tumor volume was strongly correlated with RECIST score(r=0.646,P<0.01)and moderately correlated with Choi score(r=0.498,P<0.01).Area under curve(AUC)in ROC curve was 0.847(95%CI:0.679~1.000),P<0.05.Conclusion The tumor volume is helpful to evaluate the efficacy of drug treatment in GIST patients,and has important guiding significance for the control of drug treatment time and the decision-making of operation time.
