血清CK、CK-MB、sTREM-1和PCT水平在新生儿感染性肺炎病情及感染类型评估中的价值

Value of serum CK,CK-MB,sTREM-1 and PCT levels in the neonatal infectious pneumonia and infection type

摘要 目的探讨血清肌酸激酶(creatine kinase,CK)、肌酸激酶同工酶(creatine kinase-MB,CK-MB)、可溶性髓样细胞触发受体-1(soluble myeloid cell trigger receptor-1,sTREM-1)和降钙素原(procalcitonin,PCT)在评估新生儿感染性肺炎病情及感染类型中的价值。方法选取我院2020年6月~2022年6月收治的新生儿感染性肺炎患儿90例,根据其感染类型分为细菌性肺炎组(30例)和非细菌性肺炎组(60例)两组,对比两组血清指标;根据其病情严重程度分为轻症(20例)和重症(70例)两组,对比两组血清指标。结果细菌性、非细菌性患儿CK、CK-MB、sTREM-1、PCT水平比较,差异有统计学意义(P<0.05);经受试者工作特征曲线(receiver operating characteristic curve,ROC曲线)分析显示,CK、CK-MB、sTREM-1、PCT能够用于评估新生儿感染性肺炎感染类型,曲线下面积分别为0.675、0.674、0.720、0.709,P<0.05;轻症、重症患儿一般资料比较,差异无统计学意义(P>0.05);不同病情程度患儿CK、CK-MB、sTREM-1、PCT比较,差异均有统计学意义(P<0.05);CK、CK-MB、sTREM-1、PCT水平与病情严重程度均呈正相关(P均<0.05)。结论CK、CK-MB、sTREM-1、PCT在新生儿感染性肺炎病情及感染类型的评估中具有一定价值,临床可通过监测上述指标对患儿病情诊疗提供依据。 Objective To investigate the value of serum creatine kinase(CK),creatine kinase-MB(CK-MB)soluble myeloid cell trigger receptor-1(sTREM-1)and procalcitonin(PCT)in the evaluation of neonatal infectious pneumonia and infection type.Methods Ninety children with neonatal infectious pneumonia admitted to our hospital from June 2020 to June 2022 were selected and divided into bacterial group(30 cases)and non-bacterial group(60 cases)according to their infection types.The serum indexes of the two groups were compared.According to the severity of the disease,the patients were divided into mild group(20 cases)and severe group(70 cases),and the serum indexes were compared.Results There were significant differences in the levels of CK,CK-MB,sTREM-1 and PCT between bacterial group and non-bacterial group(P<0.05).ROC analysis showed that CK,CK-MB,sTREM-1 and PCT could be used to evaluate the infection types of neonatal infectious pneumonia,and the areas under the curve were 0.675,0.674,0.720,and 0.709(P<0.05),respectively.There was no statistical significance in the general data between mild group and severe group(P>0.05),but there were statistical significance in the comparison of CK,CK-MB,sTREM-1 and PCT among children with different disease degrees(P<0.05).The levels of CK,CK-MB,sTREM-1 and PCT were positively correlated with the severity of the disease(P<0.05).Conclusion CK,CK-MB,sTREM-1 and PCT have certain value in the evaluation of infectious pneumonia disease and infection type in neonates,and the above indicators can be used to provide evidence for the diagnosis and treatment.
