内镜下冷圈套与热圈套切除术切除结肠较大息肉的效果比较

Efficacy comparison of endoscopic cold trap and hot trap polypectomy for large colonic polyps

目的比较内镜下冷圈套与热圈套切除术切除结肠较大息肉的效果。方法回顾性分析2021年6月~2022年4月本院收治的109例结肠较大息肉患者的临床资料,根据手术方法不同分为热圈套组(内镜下热圈套切除术)52例与冷圈套组(内镜下冷圈套切除术)57例。比较2组手术相关指标、手术相关并发症、疼痛情况、息肉复发情况。结果冷圈套组息肉切除时间短于热圈套组(P<0.05),冷圈套组金属夹使用数量低于热圈套组(P<0.05);术后3 d,2组血清神经生长因子(nerve growth factor,NGF)与前列腺素E2(prostaglandin E2,PGE2)水平、视觉模拟评分(visual analogue score,VAS)均降低(P<0.05),且冷圈套组更低(P<0.05);2组手术相关并发症发生率、息肉复发率比较无统计学差异(P>0.05)。结论内镜下热圈套与冷圈套切除术切除结肠较大息肉的并发症发生率相近,且内镜下冷圈套切除术疼痛不明显,息肉切除时间更短。

Objective To compare the effect of endoscopic cold trap and hot trap polypectomy for large colonic polyps.Methods The clinical data of 109 patients with large colon polyps admitted to the hospital from June 2021 to April 2022 were analyzed retrospectively.According to different surgical methods,52 patients were divided into the hot trap group(endoscopic hot trap polypectomy)and 57 patients into the cold trap group(endoscopic cold trap polypectomy).The operation related indexes,operation related complications,pain and polyp recurrence were compared between the two groups.Results The polyp removal time in cold trap group was shorter than that in hot trap group(P<0.05),and the number of metal clips used in cold trap group was lower than that in hot trap group(P<0.05).Three days after operation,the levels of serum nerve growth factor(NGF),prostaglandin E2(PGE2)and visual analogue score(VAS)were decreased in both groups(P<0.05).And the cold trap group was lower(P<0.05).There was no significant difference in the incidence of surgical related complications and polyp recurrence between the two groups(P>0.05).Conclusion The incidence of complications of endoscopic hot trap and cold trap polypectomy for large colonic polyps is similar;and the pain of endoscopic cold trap polypectomy is not obvious,and the polyp removal time is shorter.
