两种手术方式治疗翼状胬肉的临床对照研究

A comparative study of two different operation methods in the treatment of pterygium

目的探讨采取改良翼状胬肉切除联合羊膜移植术与改良翼状胬肉切除术两种手术方式治疗翼状胬肉的临床疗效差异。方法选取2017年2月~2018年5月在我院确诊并收治住院行手术治疗的120例(120眼)翼状胬肉患者作为研究对象。应用随机数表法将患者分为羊膜组和对照组,进行改良翼状胬肉切除联合羊膜移植术的病例为羊膜组60例(60眼),进行改良翼状胬肉切除术的病例为对照组60例(60眼)。观察并记录两组患者术后翼状胬肉复发情况并进行对比分析。结果对照组术后裸眼视力低于羊膜组,两组数据差异具有统计学意义(P<0.05);羊膜组角膜散光度显著低于对照组,差异有统计学意义(P<0.05);术后对照组的拆线时间和角膜上皮修复时间均明显长于羊膜组,差异有统计学意义(P<0.05);术后对照组比羊膜组的并发症发生率高,差异有统计学意义(P<0.05);复发率明显低于对照组(3.3%vs 15%),两组数据差异具有统计学意义(P<0.05)。结论改良翼状胬肉切除联合羊膜移植术相较于改良翼状胬肉切除术治疗翼状胬肉的疗效更佳。

Objective To investigate the difference of clinical effect of modified Pterygium resection combined with amniotic membrane transplantation and modified Pterygium resection in the treatment of pterygium.Methods From February 2017 to May 2018,120 patients(120 eyes)with pterygium were selected as the study object.The patients were divided into amniotic membrane group(60 eyes)and control group(60 eyes).The recurrence of pterygium in the two groups was observed and recorded.Results The visual acuity of naked eyes in the control group was lower than that in the amniotic membrane group,the difference between the two groups was statistically significant(P<0.05);the corneal astigmatism in the amniotic membrane group was significantly lower than that in the control group,the difference was statistically significant(P<0.05);the time of line removal and corneal epithelial repair in the control group were significantly longer than that in the amniotic membrane group,the difference was statistically significant(P<0.05);the time of corneal epithelial repair in the control group was significantly longer than that in the amniotic membrane group.The incidence of complications was high and the difference was statistically significant(P<0.05);the recurrence rate was significantly lower than that of the control group(3.3%vs 15%),The difference between the two groups was statistically significant(P<0.05).Conclusion The effect of modified pterygium excision combined with amniotic membrane transplantation is better than that of modified pterygium excision.
