摘要
目的观察维持性腹膜透析(maintenance peritoneal dialysis,MPD)患者血清半乳糖凝集素3(galectin-3,Gal-3)的表达情况,并探讨其与心血管疾病(cardiovascular disease,CVD)的相关性。方法选取MPD患者80例及健康对照组40例为研究对象。收集患者年龄、性别、有无CVD病史等临床信息,根据有无CVD发生,分为CVD组和无CVD组。检测血清Gal-3、血红蛋白(Hb)等指标;并记录心脏彩超检查结果。结果(1)MPD组血清Gal-3水平明显高于健康对照组;(2)以年龄校正后,血清Gal-3与Hb、舒张压呈正相关,与血清白蛋白(Alb)、残余肾功能呈负相关;(3)CVD组血清Gal-3、Hb、血磷、左心房内径(left artrial diameter,LAD)、左心室舒张末期内径、室间隔厚度明显高于无CVD组,而Alb、左心室射血分数明显低于无CVD组;(4)多因素Logistic回归显示,Gal-3、Hb、Alb、LAD值是MPD患者发生CVD的独立危险因素。结论MPD患者血清Gal-3水平明显升高,血清Gal-3、Hb、Alb、LAD值是MPD患者发生CVD的独立危险因素。
Objective To determine the level of serum galectin-3(Gal-3)and its correlation with cardiovascular disease(CVD)in maintenance peritoneal dialysis(MPD)patients.Methods Eighty MPD patients and 40 healthy controls were recruited.Control group which matched with age and sex of MPD were selected from medical examination center.Age,sex,with or without CVD history,hemoglobin(Hb),serum Gal-3 and echocardiography were recorded.According to CVD history,the patients were divided into CVD group and non-CVD group.Results(1)Serum Gal-3 in MPD group was significantly higher than healthy controls.(2)After adjustment for age,serum Gal-3 in MPD patients was positively related to Hb and diastolic pressure,and negatively related to serum albumin(Alb)and residual renal function.(3)Compared with non-CVD group,patients in CVD group had significantly increased Hb,serum phosphorus,serum Gal-3,left artrial diameter(LAD),left ventricular diameter,and interventricular spetum,but significantly reduced Alb and left ventricular ejection.(4)Multivariate Logistic regression analysis showed that Hb,Alb,LAD,and serum Gal-3 were independently correlated with CVD in MPD patients.Conclusion Serum Gal-3 in MPD patients is significantly elevated.Serum Gal-3,Hb,Alb and LAD are independent risk factors of CVD in MPD patients.
