摘要
目的探讨C-反应蛋白、红细胞沉降率及降钙素原与消化性溃疡出血程度的相关性。方法选择2019年8月至2020年1月因消化性溃疡并出血到哈尔滨医科大学附属第一医院消化内科病房住院患者80例。将患者在胃镜下进行Forrest分级,分别纳入ForrestⅠ级组、ForrestⅡ级组、ForrestⅢ级组,并进行CRP、ESR及PCT检查。比较三组患者间CRP、ESR及PCT阳性率及数值的差异。结果ForrestⅠ级、Ⅱ级及Ⅲ级者CRP的阳性率分别为54.5%、17.9%、13.3%,其中ForrestⅠ级分别与Ⅱ级和Ⅲ级对比P值为0.023、0.013(P<0.05);ForrestⅠ级、Ⅱ级及Ⅲ级者CRP的中位数分别为8.37、3.82及3.19 mg/L,其中ForrestⅠ级分别与Ⅱ级和Ⅲ级对比P值为0.045、0.010(P<0.05);不同Forrest分级间的ESR和PCT的阳性率及数值无明显差异(P>0.05)。结论CRP可作为评估消化性溃疡出血程度的观察指标,ESR及PCT不能作为评估消化性溃疡出血程度的观察指标。
Objective To investigate the correlation of C-reactive protein,erythrocyte sedimentation rate and procalcitonin with bleeding degree of peptic ulcer.Methods From August 2019 to January 2020,80 inpatients with peptic ulcer complicated with hemorrhage in the Department of Gastroenterology of the First Affiliated Hospital of Harbin Medical University were selected.Patients were divided into ForrestⅠ,ForrestⅡand ForrestⅢgroup during gastroscopy,and underwent CRP,ESR and PCT examinations.The positive rates and values of CRP,ESR and PCT were compared among the three groups.Results The positive rates of CRP in ForrestⅠ,ⅡandⅢwere 54.5%,17.9%and 13.3%,respectively,and the P values of ForrestⅠcompared withⅡandⅢwere 0.023 and 0.013,respectively(P<0.05).The median values of CRP in ForrestⅠ,ⅡandⅢwere 8.37,3.82 and 3.19 mg/L,respectively,and the P values of ForrestⅠwere 0.045 and 0.010,respectively,compared with those of ForrestⅡandⅢ(P<0.05).There was no significant difference in ESR and PCT positive rates and values between different grades(P>0.05).Conclusion CRP can be used as an observation index to evaluate the degree of bleeding activity in peptic ulcer.However,ESR and PCT could not be used to evaluate the degree of bleeding activity in peptic ulcer.
